We are currently looking for an enthusiastic and highly motivated Registered health care professional to join our highly professional and motivated directorate to support the development and delivery of an Enhanced Care and Enhanced Care Activity Room service. The post holder will be supported and managed by Care Group 1 (healthy lives) and work with discharge team to support discharges of patients.

Main duties of the job

The Enhanced Care and Activity Room Coordinator will be an autonomous practitioner responsible for the assessment and subsequent planning of care for patients presenting with complex health and social needs.

They will be responsible for coordinating an integrated, comprehensive and person-centred service, working collaboratively with multi-disciplinary team members to ensure a high and consistent standard of both ward based enhanced care and activity room enhanced care is delivered to individual patients across the Trust to optimise patient outcomes.

This post is firmly based in clinical practice and involves working directly with patients, carers and multidisciplinary teams.

Job responsibilities

Job Purpose
  • They will work alongside the enhanced care co-ordinator/Discharge co-ordinators to provide a cross over service into enhanced care and discharge assessments when required.
  • This post is firmly based in clinical practice and involves working directly with patients, carers and multidisciplinary teams. The remaining time will be utilised in education, audit, research, leadership and service development.
  • They will manage a team of Enhanced care workers who are employed to provide enhanced care to patients who require increased supervision and support due to complex health and social needs.
  • They will be responsible for coordinating an integrated, comprehensive and person-centred service, working collaboratively with multi-disciplinary team members to ensure a high and consistent standard of therapeutic care is delivered to individual patients across the Trust to optimise patient outcomes.

  • Key Result Areas
  • To coordinate a specialist service for patients who have complex health and social needs. Complete relevant assessments required and allocated enhanced care workers to patients.
  • Supervise a team of enhanced care workers.
  • To assist in the identification and development of clinical protocols and strategies to support and enhance the continuity and standard of enhanced activity room whilst ensuring equity of access to the service.
  • To identify, implement and maintain a suitable mechanism for referrals to the Enhanced Care Activity Room.
  • To provide expert advice and support to patients, their families and other healthcare professionals throughout a patients admission and stay in hospital.
  • To be actively involved in in-patient assessment for suitability and access to the enhanced care activity room, liaising with ward staff and other health care professionals to ensure high quality care is provided when required.
  • To ensure that patients and their families are fully informed and supported by providing appropriate information and guidance.
  • To liaise with the ward staff to ensure and support appropriate and timely assessment and planned care plans for patients, for example Mental Capacity Assessment (MCA) and Depravation of Liberty (DOLs).
  • To develop integrated care plans for patients referred to the Enhanced Care Activity Room for intervention and support.
  • Promote close working relationships with, for example, Registered Nurses, Ward Matrons, Medical staff, Specialist nurses, Therapists and the Adult Vulnerability Team.
  • To actively involve service users in providing feedback of their experience of the current service and suggestions for improvements.
  • To participate in relevant work at Directorate level to represent the service perspective.
  • To assist in the development of evidence based clinical nursing practice in line with current research and guidelines.
  • Assist in the audit of service provided against clear outcome measures and report those outcomes monthly.
  • Contribute to the on going development and expansion of services in line with National and Local quality initiatives.
  • Promote a culture of respect, dignity and informed choice for persons with complex health and social needs, through advocacy and support in decision-making.
  • Organise and deliver training and education in the care and assessment and provide support to nursing staff.
  • Use of the dementia activity computer system to facilitate games, movies and activities.

  • Communication and Working Relationships
  • Ability to use highly developed communication skills including tact, diplomacy, discretion, empathy, negotiation skills, motivational skills, persuading and influencing skills and counselling skills.
  • A requirement to communicate and understand complex information in potentially emotionally charged and hostile situations and be confident to challenge this information when required.
  • Communicate complex information to groups of staff e.g. Ward based Multidisciplinary teams.
  • Work in partnership with all clinical directorates to foster a positive climate of relationships to manage the delivery of the Enhanced Care Activity Room Service.
  • To inspire and motivate, in partnership with patients, carers, staff and Ward Matrons.
  • To coordinate a specialist service which delivers high quality and consistent care to patients who have complex health and social needs.
  • Ensuring that timely, accurate and consistent advice and support is provided to staff within the Trust.
