- To work in collaboration with Primary Care Mental Health ServicesManager and team leader colleagues, to ensure the highest standards of practice and service delivery.
- To work as part of the Strategic Business Unit to deliver the services required under the Service Specification for Primary Care Mental Health Services.
- To provide day to day operational management, leadership, and supervision to Primary Care Mental Health Services Staff.
- The Team Leader will manage primary care mental health staff which may include EPMHS staff and GP+ staff within the designated community mental health service ensuring that the needs of service users are met through the effective deployment of these staff. They will also maintain a clinical leadership role and/ or a caseload dependent on sector need.
- The Team Leader will contribute at a local level to the development and implementation of policies and procedures.
- The Team Leader may on occasion deputise for the Primary Care Mental Health Services Manager in his or her absence, as agreed with the Service Line Leader.
Main duties of the job
- To plan, prioritise and manage the delivery of the Enhanced Primary Care Mental Health Service within the designated community mental health service area, including overseeing referrals and ensuring that there is an even and appropriate distribution of work across the team, based on the experience of team members.
- To ensure that risks relating to service users, unmet needs and to the work environment are assessed and managed appropriately, ensuring that all team members know and understand their role in risk management.
- To implement and monitor relevant operational policies and protocols, team systems and processes for the effective operation of his/her team.
- To monitor the service caseload ensuring that the teams activities are targeted to clients/patients, fulfilling the agreed criteria.
- In collaboration with the Service Manager, to undertake the timely recruitment selection, appointment and induction of team members.
- To manage the delegated team budget with respect to operational and service delivery costs.
- To effectively manage all aspects of the team's time i.e. Rota, Time worked, Annual Leave via the E Roster system.
- To act for or on behalf of the Community Mental Health Service Manager as required from time to time in accordance with local agreements
- Where the Team Leader is an Approved Mental Health Professional to be prepared to participate in local AMPH rota where appropriate.

- Ensure the provision of appropriate triage and advice to individuals with severe mental illness and complex needs, presenting in primary care.
- Ensure the liaison with other services or agencies to support the primary care network response to mental health needs in primary care.
- Participate in risk assessment and risk management activities appropriate to primary care.
- Maintain service activities in line with the agreed referral and triage criteria.
- To provide a flexible service, responsive to Service User need and Stakeholder feedback.
- To develop the service in line with Trust strategy
- Maintain an effective skill/ professional personnel mix to provide a multi-professional team approach.
- Put in place systems to investigate and access a wide range of services designed to meet occupational, recreational, social and health needs in primary care.
- To provide information for service development and evaluation through the use of outcome measurements, audit and annual service policy review.
- Implement risk assessment procedures related to the working environment, tasks and client management.

Qualifications
Essential
- RMN/DipSW/CQSW/OT qualification.
- Evidence of current registration with the NMC, GSCC or HPC.
Desirable
- Cert or Dip. in management.
Experience

- Extensive post qualifying experience of working with people with mental health problems in the community.
- Experience of working within or with primary care services
- Experience of providing clinical and management supervision.
- Involvement in the development of a new service or of change management.
