We wish to recruit a band 5 Staff Nurse to join our integrated Locality Teams in Gateshead.

You will work for a forward thinking Trust, in a dynamic and transformational business unit, committed to working within a cycle of continuous improvement, with the patient at the Centre of all you do. The locality teams are multi professional working in collaboration to ensure safe, highly effective patient care.

This post will provide an exciting opportunity to work differently within an new initiative to deliver a high quality service to patients in Gateshead.

You will need to be motivated, organised and flexible. The service core hours are 8am - 4:30pm but we do cover the service 8am - 8pm, 7 days per week. You will be expected to work flexibly to deliver service needs.

You must be a registered nurse and evidence of a teaching certificate, post registration experience and ongoing professional development are desirable.

It would also be desirable to have evidence of experience in a community setting.

You must be a car owner/driver.

Full and part time hours available.

Main duties of the job

The delivery of high standards of clinical practice that hold patients and their families at the center to ensure that they receive safe, quality care that gives them a good experience.

To be responsible for delivery of an identified package of care under the supervision and direction of Specialist Nurse Practitioner/Senior Nurse/Locality Team Lead.

Job responsibilities

To provide high quality patient care in the community setting under the direction and supervision of a senior nurse/practitioner.

To support the assessment of the health and health related needs of patients, clients, their families and other carers.

To assess, plan, provide and evaluate nursing care to meet care needs of individual patients and carers in their own homes.

Person Specification



  • Diploma/Degree in Adult nursing
  • NMC registration


  • Degree or willing to work towards
  • Teaching qualification
  • IT qualification



  • Portfolio of professional development post registration
  • Ability to organise workload
  • Ability to prioritise appropriately
  • Ability to work under pressure and achieve tight deadlines in a complex/changing environment


  • Experience of working in a multiprofessional team
  • Leadership and management experience
  • Working with the frail elderly
  • Previous Community experience/ NHS
  • Experience of undertaking research and audit
