Are you a passionate, innovative and inspirational individual who is looking for a unique opportunity to support the implementation of our vision for a multi-agency discharge and community hub?
The Acute Response and Rehabilitation in the Community and Hospital (ARCH) have recently secured significant additional resource, which will allow the service to expand the cohort of patients who can be identified to leave hospital sooner with additional short-term support in their home environment. This aims to reduce the need for inpatient rehabilitation, risk of deconditioning and interim placements.
Our ARCH rotation provides the opportunity for successful applicants to rotate between two core areas, to develop competencies that support positive patient outcomes, 7 days a week. The rotations across two key areas include- virtual ward, including community rehabilitation and hospital in reach, including the emergency department and same day emergency care. This provides varied opportunities across the hospital base and community and you will get the chance to work in all of the areas.
What do we offer you in return?
Successful candidates will be expected to partake a 7 day service covering 8am to 8pm across all rotational areas.
