We are looking for a caring, motivated friendly community nurse to join our busy, professional and dynamic team. You should possess commitment to interdisciplinary team working and innovative delivery of excellent standards of care. You will be able to demonstrate a real enthusiasm for community nursing, as well as a desire for on-going professional development.Main duties of the job
The team provides nursing care to patient within their own homes or chosen place of residence offering an alternative to hospital admission, supporting timely discharge and enabling people to realize their potential.

Able to assess, plan, implement and evaluate interventions, liaising withcolleagues and partners to ensure an integrated approach to the needs of the service user.
To organise and deliver skilled clinical support work without direct supervision. This will include individual in a variety of settings.
Gain valid informed consent and work within legal frameworks with patients.
To liaise with senior clinicians with regard to patient care, highlighting anyvariations to expected outcomes from agreed treatment programmes. To provide basic verbal and written clinical reports and document patientinterventions/findings in case notes in line with professional body and Trust standards.Person SpecificationTraining and QualificationEssential
- RGN Qualification
- NMC Registration
- Ability to demonstrate a level of English language and grammar commensurate with writing in service user case notes
Knowledge and SkillsEssential
- Previous community experience
- Post registration general experience
- Previous NHS experience
- Experience of working in a multi disciplinary / multi agency health/social care environment.
- Experience and knowledge of basic administration systems including Microsoft office
- Experience of successful partnership working arrangements with other services
