We have an exciting new opportunity to join a new role as a primary care mental health practitioner working within a Primary Care Network (PCN) in Bradford. We are looking to recruit Band 6 mental health professionals (Nurse, Social Worker, OT).
The introduction of the primary care mental health practitioner role aligns with the wider community mental health transformation and expansion of community-based mental health support services as set out in the NHS Long Term Plan. The transformation aims to bridge the gap between primary care and secondary care services.
The role supports improved access to mental health support, helping people consider achievable goals, access local community organisations/resource and promotes self-management.
Practitioners will be embedded in PCNs, collaborating with primary care colleagues to deliver person-centred, holistic assessments and interventions in local GPs. The role may also form part of multi-disciplinary meetings held by GP practices within primary care. Practitioners will offer a link between primary and secondary care services for people with mental health needs.
The role will link to a specific primary care network and work closely with the practices aligned to that network. The successful candidate will be based in GP sites within the PCN during working hours.
Clinical and management supervision is provided via BDCT, offering support and opportunities for continued professional development.
Main duties of the job
- To actively promote better understanding and knowledge of mental health and wellbeing within primary care to enable the MDT to provide integrated and holistic offers of support.
- To take a lead on providing specialist assessment of service users with mental health problems within the primary care setting.
- To provide effective signposting based on a holistic assessment of an individuals needs making use of a range of resources and services including VCSE and community based organisations
- To provide evidence based, time limited interventions where appropriate and within scope of professional practice and experience
- When required, to facilitate swift and seamless transition into other mental health services and pathways
- To work collaboratively with service users and their families to:a. support shared decision-making around their care and support working on strengths and assets b. promote recovery and self care
- To inform and influence the ongoing development of new and innovative models of support within primary care and secondary mental health service

- Previous experience of working with and holding a caseload of indivduals with serious mental health problems
- Previous experience of working autonomously assessing, developing care plans and managing risk within a community setting
- Previous experience of working collaboratively within an MDT
- Previous experience of service development, quality improvement and of bringing innovation and new ideas to practice
- Previous experience delivering biopsychosocial mental health interventions
- Previous experience of co ordinating care and signposting to relevant agencies and services
Qualifications
Essential
- Registered Professional in either Nursing (Mental Health), Social Work or Occupational Therapy, with current registration with NMC, Social Work England or HCPC.
- Educated to degree level or equivalent experience
