If so, we would love you to join our Central Manchester Community Response team.

We are looking for an Advanced Clinical Practitioner who will work within a community based MDT, sharing a passion for supporting patient care and enthusiasm for making a difference. The ACP will work on a rotational basis between our Hospital at Home and Urgent Care Crisis Response Team.

The Hospital at Home service enables patients to receive safe, effective treatment at home for up to 14 days. This provides patients, who are not yet medically fit, with support, choice, and inclusion at every stage of their care, including hospital discharge.

They will provide medical assessments at home alongside our GPs, with overall care under a hospital consultant.

Our MDT collaborative Crisis Response Team delivers an Admission Avoidance aspect of the service. We receive referrals from community services, NWAS, 111, GPs and ED deflections from rapid response.

A confident communicator with strong organisational skills, you will thrive in an MDT environment and will be instrumental in supporting and developing this new service, to ensure a high-quality service for our patients.

You will be involved in team projects and trust initiatives and will receive regular supervision, in-service training, and support, to pursue post graduate education.

Main duties of the job

You will also have the opportunity to work across hospital and community whilst developing key links and networks with acute and community colleagues. This will enable you to use your specialist clinical skills across all four pillars of advanced clinical practice to determine the most appropriate pathways for your patients and will include access to virtual patient monitoring for patients that would benefit from additional clinical oversight.

You will work closely with the multi-disciplinary team across acute, community and voluntary partners to ensure discharges are safe and effective and to prevent re-admission.

Using your experience and knowledge, you will support other team members to ensure we deliver a high-quality service for our patients, providing education and support as needed.

You will be involved in both team projects and wider trust initiatives giving you the opportunity to form close professional relationships with a wide variety of multi-disciplinary team members.

Qualifications

Essential

• Current registration with NMC, HPC and or other statutory body
• Relevant professional health degree

Experience

Essential

• Substantial post registration experience. Leadership. experience at Band 7 or above
• Proven experience of working as an independent practitioner
• In-depth knowledge of recent NHS or Equivalent health care legislation, relevant to community
• Experience of teaching and mentoring of staff across the multi-disciplinary team.

• Able to demonstrate care which reflects evidence based practice.
