The post holder will have responsibility to provide a single point of access for all referral into Adult Mental Health services.
To continue to review all referrals to identify any gaps in the referral process with a view to strengthening existing joint partnership systems to improve early and appropriate referrals.
To collate and analyse the statistical data from the current database to facilitate the process.
To triage all referral information, making autonomous clinical judgement in order to prioritise and allocate the referral to the appropriate professional within our service or suggest alternatives to the referrer. This is complete in line with the Mental Health Triage Scale, risk management process, the Mental Health Measure (Wales) 2010, and the UK Mental Health Triage Scale Guidelines (2015)
Triaging phone calls from external agencies and the general public. Provide brief interventions during the telephone calls.
Conducting holistic assessments. Formulating risk assessments.
Main duties of the job
An exciting opportunity has arisen within the Adult Mental Health Service. The opportunity involves being part of a developing and expanding team to improve the access and response to mental health services.
The post is for a dedicated Single Point of Access Professional. The job is open to qualified mental health professionals
The post will require the Professional to Triage all referral information, prioritise and allocate to appropriate professionals/services. The role will also include face to face assessments.
The single point of access is a new developing team within the health board. The team will encourage development and support. The ethics of the team is to deliver high quality care and support to individuals experiencing mental health concerns.
The ability to speak Welsh is desirable for this post English and/or Welsh speakers are equally welcome to apply.

- RMN registration, occupation therapy/social work
- Possession of post graduate qualification (diploma, degree or higher qualification) or evidence of working towards the same.

- Substantial post qualification experience as and Band5/6 or equivalent working within the field of acute adult/ community mental health.

- Experience of teaching and training other health and / or social care professionals and designing educational inputs (this could include conference presentations, publications etc).
