Due to huge success and expansion in the role of coordination community urgent care referrals, the Rapid Response Coordination Centre is seeking an additional team member to help grow this role and service.
The Coordination centre is responsible for the triaging of referrals to community urgent care services, Rapid Response and Urgent Crisis Response teams and coordinating them to see ensure patients are seen against service time frames. Following a current expansion, we are looking for TWO band 6 clinical team members to work during peak referral times (predominantly Mon-Fri with weekend working required). Note the Coordination centre operates between 08:00-21:00 seven days a week.
