An exciting opportunity has arisen for a suitably qualified and experienced Registered Nurse to work with the Palliative Day and Out of Hours Team. We are a small, friendly team who are supportive of one another. Education, development and progression is encouraged and supported and opportunities to develop are offered in line with appraisal and personal development plans.

The Day and Out of Hours Team are part of the wider Multi-Disciplinary Team and are supported by our Specialist Nurses and Palliative Care Consultants to provide 24 hours palliative and end of life care to patients and their significant others to enable preferred place of death to be achieved.

Main duties of the job

Part time hours are available, working shift patterns include day, evening and Night Shift where required.

The main duties of the role are:

To lead and coordinate the delivery and development of the palliative care service through the use of effective evidence based practice in line with agreed national standards for patient safety and clinical practice

To provide the highest standard of holistic care for patients with advanced life limiting illnesses and their carers through the utilisation of specialised clinical care and advice in the assessment and management of patients, working autonomously and with the multi-disciplinary team to improve the quality and safety of patient care

To work as part of an Integrated team and be accountable for the planning and delivery of all aspects of palliative and end of life care as set out in the NMC code

To continuously improve clinical care standards in line with the clinical governance framework to ensure the highest quality of care is provide in the safest manner

To provide clinical leadership, expertise and specialist clinical advice and support to clinical staff on the management and care of patients within palliative care and lead the effective delivery of care to patients

To lead in the provision of expert specialist palliative care knowledge, care and experience to support the wider team, facilitating the effective delivery of care to patients at all times

The post holder will work as part of a Community Team alongside the Nursing Sister/Charge Nurse

To provide the highest standard of holistic care for patients with advanced life limiting illnesses and their carers through the utilisation of skilled palliative care within patients own homes which may include Nursing and Residential Care Homes and establishments.

The post holder will be expected to provide appropriate emotional support to patients, carers and families working under the direction of the Nursing Sister/Charge Nurse and be able to assess the holistic needs of patients and be able to plan, implement and evaluate care and treatment plans.

The post holder will have the ability to work collaboratively as part of wider integration within Community Services, as well as be able to contribute to multi-disciplinary Team working to ensure practice is efficient, effective and evidence based.

The post holder will be expected to work flexibly to meet the needs of the specialist Palliative Care Service and to be available to work evening and night shift as required, and to cover the Sunderland area if required.

Essential

• Long standing history of end of life care/palliative care environment at senior level

Qualifications

Essential

• Current NMC Registration
• Degree in nursing studies or healthcare related subject.
• Evidence of Post Graduate studies in Palliative Care/oncology/related subject

Desirable

• Clinical Skills
• Advanced Nurse Prescribing.
• Evidence of Research/Audit

Physical Skills

Essential

• Ability to assist patients to move as per Trust Moving and Handling Policy

Skills and Knowledge

Essential

• Ability to provide and receive complex, sensitive or contentious information
• Able to work without supervision
• Competent in the use of IT such as Microsoft Office
• Evidence of CPD
• Knowledge of national health and social care policy relating in particular to the Regional and National Palliative and End of Life Care agenda
