An exciting opportunity has arisen for a Community Mental Health Practitioner within our friendly and supportive team at South Lakes Community Mental Health Team. In this role, you will be a pivotal part of our multi-disciplinary team.
You will be a Mental Health Nurse, Social Worker or Occupational Therapist with current professional registration with experience of supporting those with mental condition within the community.
You will be based in Kendal and will cover a geographical area within the South Lakes locality. The service is operational Monday to Friday between the hours of 9.00am and 5pm.
As part of the multi-disciplinary team you will undertake comprehensive initial assessments for service users with high complex needs who have been referred to the Community Mental Health Team. You will hold a caseload of Service Users and provide treatment and planned interventions.
Part of your role will require you to undertake the role of team duty worker. This role involves managing unplanned calls relating to service users on the CMHT caseload. This may be service users who have an allocated worker who is on leave/training that day.
Main duties of the job
You will have current or previous experience of working within a mental health setting. Clinically confident in working with individuals with complex needs and be able to work effectively in a multi professional context. You should have a collaborative and enthusiastic approach as well as passion and commitment in developing and promoting recovery based approaches for adults with severe and enduring mental health needs.
