The successful applicants will be based in the integrated community mental health teams in Ealing as part of the community and Recovery Mental Health Services service line. The service comprises a fully integrated mental health team with a visible presence in the local Primary Care Network.
There will be a tiered or stepped care model within the MINT team where a more primary care ethos and model of delivery will be used for the majority of patients but those who have more complex needs will have more enhanced support from an allocated clinician as well as the multidisciplinary team. The post will therefore involve consultation/liaison with the MDT as well as GPs, primary care services and other mental health teams. Alongside this there will be some direct consultation with patients particularly around complex cases and crisis prevention.
Our implementation of MINT has taken a whole-systems approach to the way Mental Health care is delivered with transformation at every level of service and care delivery.
This has involved integrating primary and secondary care teams and reconfiguring community teams so they are positioned to deliver care in alignment with the local primary care networks and work in partnership with local communities.
Main duties of the job
MINT aims to deliver joined-up, personalised treatment and care which support and are responsive to, the needs of the local population. MINT teams are place-based aligning primary care networks across the boroughs to deliver an integrated approach to mental health, physical health and social care needs.
Operational as of July 2021, MINT provides community-based mental health services and support for adults aged 18+ with a wide range of mental health difficulties living in the London boroughs of Ealing, Hounslow and Hammersmith & Fulham.
MINT offers a range of therapeutic interventions which support and are tailored to individual needs. These are delivered in both group and one-to-one settings. The services and support available throughout MINT differ across each borough, reflecting the needs of the local population in each area. A stepped approach to care is taken, with brief group interventions offered as a starting point ensuring a range of accessible treatments including therapeutic support, education and training, employment, volunteering and other occupational support, peer support, and link workers building community connections.
About us
The MINT teams are an exciting, vibrant and exciting place to work. There are opportunities to get involved with additional research, service development, training or to develop clinical specialisation. The trust actively promotes CPD and is a valuing and rewarding employer.
The Trust is committed to supporting the continued health and wellbeing of our staff, which has been an area of significant focus during the pandemic. Our ambition is to build on the learning from this and develop a sustainable health and wellbeing offer that represents excellence as an employer. The Trust provides free access to a number of support tools including our Health at Work team which consists of Occupational Health professionals including specialist services such as staff counselling, incident de-brief and physiotherapy.
Health and wellbeing conversation have been integrated into supervision, appraisal and reflective practice discussions. In addition, there are a number of COVID specific support tools currently in place e.g. free access to wellbeing apps and psychological support. All staff are encouraged to get the Covid vaccine. The Trust is currently in a period of re-set and recovery and is supporting teams to come back together in a safe and secure way.
