Opportunities have arisen for two dynamic individuals to join us as Advanced Clinical Practitioners. The post(s) support the Acute Medicine department and the Modernising Medicine transformation programme, which will build upon areas of good practice at King's and draw on innovative practices at a regional and a national level. The transformational context of this role presents a unique opportunity to co-design and deliver best practice in acute and general patient care delivery. This is a dynamic department with a strong track record in delivering innovative pathways of care, high quality multi-professional training and academic research.
The successful candidate will join and support a friendly and engaged team with responsibility for the management of acutely ill medical patients. Your role will involve assessment, treatment and management of a diverse undifferentiated caseload of patients requiring urgent care. This may include establishing early plans for future management through ambulatory emergency care or hot clinics, admission to and discharges from our AMU or a referral to specialist clinics. You will work closely with our colleagues from Emergency Medicine and other medical sub-specialties and with primary care and community services. The post holder will contribute to the delivery of a seven-day acute care service.
Main duties of the job
The Advanced Clinical Practitioner role is within Acute Medicine. The post holder will bring a high level of expertise within acute medical services providing advice, education and support to staff, patients, their families, and carers.
The post holder will have the capability to practise autonomously as an advanced clinical practitioner within SDEC (Same Day Emergency Care) and the medical assessment unit to provide patient-centred clinical care. This will encompass the skills of advanced clinical assessment, examination, diagnosis and treatment within an agreed scope of practice

Clinical
Work autonomously as an advanced practitioner within acute medicine, managing a caseload of patients delivering individualised, direct patient care. Demonstrate a portfolio of ACP clinical competencies linked to Acute Medicine, and should be willing to merge their clinical portfolio to the RCP portfolio once it is ratified this year. Direct responsibility for assessment, examination, investigation and diagnosis of patients within acute medicine, including ambulatory care/SDEC (Same Day Emergency Care). Appropriately treat patients, resulting in the safe management and appropriate referral or discharge of patients with undifferentiated and undiagnosed presentations. Receive referrals via a variety of sources, including general practice and the Emergency Department. Manage own caseload and clinical priorities according to agreed protocols and working practices.
Leadership
Provide highly visible and accessible professional leadership within acute medicine and demonstrate expert knowledge and standards of clinical practice Lead and develop a defined area of Advanced Clinical practice within acute medicine, promoting interdisciplinary team and collaborative working practices Meet regularly with team members as a team and on an individual basis as required to support their personal and professional development.
Education and Training
Act as an expert educational resource for clinical staff, patients, and carers by providing formal and informal education. Promote and develop a positive learning environment throughout the Trust. Act as mentor/ assessor to staff members and students as required, providing educational advice and support. Ensure wards / departments can access appropriate training support and guidance. Lead and actively participate in the delivery of educational programmes for all grades of nursing, medical and allied health professional staff and for pre and post registration students
