Young Barts Health @ RLH, in partnership with Roald Dahl's Marvellous Children's Charity, has an exciting and unique opportunity to work as a Roald Dahl CMC Nurse Specialist. As the role is the first of its kind nationally, this expert practitioner will be part of a new, innovative, and evolving area of paediatric care. In addition, you will receive support from Roald Dahl's Marvellous Children's Charity including training opportunities and peer support from the network of Roald Dahl Specialists.
The Roald Dahl CMC Nurse Specialist would enable proactive and inclusive services, centered on the needs of the child and family, to prevent hospitalisation, improve communication, improve resilience, enhance relationships between families and professionals, and to have greater information sharing across organisational boundaries.
This role will suit a registered Children's Nurse with experience caring for children with medical complexity and passionate about ensuring the optimal quality of life these children deserve.
Main duties of the job
The Children with Medical Complexity Nurse Specialist (CMCNS) supports the most medically complex children from hospital to home. Children with medical complexity have multiple professionals involved in their care and the CMCNS role is to ensure that these children receive holistic care that meets their physical, social and health needs. The aim of the service is to improve the experience of families living with medical complexity and to maintain the safety and wellbeing of children who have complex medical needs.
The CMCNS will act as a key worker for families, assisting them to navigate the services and support available to them and taking the lead in coordinating their care. In addition, the expertise provided by the CMCNS will ensure that appropriate, personalised care can be delivered in the right place by the right people.
Children supported by the CMCNS will include those aged 0-18 who have a long term medical condition that involves two or more specialities as well as experiencing challenges in one or more of the following areas:
Multiple medical appointments.
Challenging social circumstances (eg.housing, safeguarding, immigration).
Children who are looked after.
Ongoing care needs in the community.
Technology Dependent care.
