Would you like to be part of an exciting new initiative?
BOB (Berks, Oxford & Bucks) Integrated Care System has been awarded new NHS funding to transform community mental health services for people with serious mental health problems. A ground-breaking new model is being implemented to benefit those primary care patients who experience serious mental health problems.
The new funding will enable 15 new Primary Care Network mental health teams to be developed. Each Primary Care Network, comprising a population of circa 30 - 50,000 patients, will host a multi-disciplinary team, which will work in close collaboration with other primary care and mental health services and clinicians, such as GPs, Specialist Mental Health Services, Social Care and Voluntary and Charitable providers to meet the needs of people with serious mental health problems.
In this role you will be working with the multi-disciplinary primary care mental health teams within a defined geographical location. You will be well supported, and will work within a dynamic, people-centred environment, where you can play a part in shaping and improving provision for patients with complex conditions. The team ethos is the use of a personalised approach to patient care that maximises independence, choice and control, following our philosophy of enabling people to reach and maintain their optimum level of health and wellbeing.
