We are looking for a dynamic and motivated practitioner to join our core community mental health team. Working as dedicated practitioner to the primary care network (PCN) you will be part of a multiagency team alongside mental health treatment practitioners, psychology, psychiatry and social care.

The successfulcandidate will provide a combined consultation, advice, triage and liaison function.
  • Work with patients to:

support shared decision-making about self-management

facilitate onward access to treatment services

provide brief psychological interventions, where appropriate.
  • Work closely with other PCN-based roles to help address the potential range of biopsychosocial needs of patients with mental health problems. This will include the PCNs MAT, including, for example, PCN clinical pharmacists for medication reviews, and social prescribing link workers for access to community-based support.
  • Operate without the need for formal referral from GPs, including accepting some direct bookings where appropriate, subject to agreement on volumes and the mechanism of access between the PCN and mental health team.


Main duties of the job

  • Work closely and co-operatively with others to ensure safe and timely provision of evidence based interventions.
  • Working within a locality service and through participation in the local referral and care pathway, provide clinical assessment and appropriate triage with a view to risk assessment, risk management, safeguarding, sign posting, treatment or onward referral in response to identified patient need.
  • Provide front line assessment to clarify the service most beneficial to the patient. This assessment may be undertaken jointly with other statutory or non-statutory members of staff.
  • Following assessment, where indicated, provide treatment using effective, evidence based brief interventions as appropriate.
  • Work closely with others, providing advice and guidance, where required, along with a more in-depth holistic mental health assessment if this is indicated. This will include a care plan linking patients into additional services, if required. The aim being to support people to address issues that may be impacting on the effectiveness of the therapy that they are receiving.
  • Promote inclusion of patients, carers, families and communities in all areas of practice.

Person Specification

Knowledge

Essential

  • Knowledge of principles of assessment of people presenting with mental health concerns
  • Knowledge of principles of risk assessment and management
  • Knowledge of Mental Health Act 1983 and Mental Capacity Act 2006 and other relevant legislation


Desirable

  • Awareness of community structures and social care management responsibilities
  • Understanding of recovery principles in Mental Health


Experience

Essential

  • Experience of working in a mental health setting
  • Assessment of people presenting with mental health issues
  • Assessment and management of clinical risk
  • Working within a multi-disciplinary team


Desirable

  • Working in a primary care setting
  • Working in collaboration with VCSE services
  • Training/developing staff and or patients
  • Experience of delivering evidenced based psycho/social interventions


Qualifications

Essential

  • Hold a Registered Mental Health Nurse, Occupational Therapist or Social worker Professional qualification
  • Educated to Degree level or equivalent experience
  • Diploma level specialist training or equivalent experience
  • Evidence of continuing professional development


Desirable

  • Teaching/training qualification
  • Psychological therapy qualification
