The Advanced Clinical Practitioner (ACP) role, as part of a multi-disciplinary team, supports in the delivery of high-quality care to those living with frailty in care homes, through comprehensive geriatric assessment, medication review and advance care planning as well as education and training.
The post holder will provide clinical expertise for developing and enhancing the service and will demonstrate advanced skills and competencies, ensuring evidence-based practice is inherent in all aspects of care and treatment. Working within local and nationally agreed protocols and guidelines, the ACP exercises independent judgement to assess, investigate, diagnose, plan, implement and evaluate the clinical care and management of frail patients in care homes.

Clinical
- Deliver and participate in the clinical care of care home residents living with frailty.
- Provide skilled assessment of patients including history taking, physical assessment,
- Prognostication, medication review and advance care planning.
- Use expert and specialised clinical and theoretical knowledge in order to safely and appropriately assess and diagnose patients in their care.
- Act autonomously within evidenced based guidelines to plan and implement individualised management plans. Formulate clinical decisions and often complex treatment plans to manage illness and/or injury.
- Demonstrate competence, knowledge and skills in a wide range of practical procedures
- Prescribe as an Independent Prescriber, assessing and prescribing appropriate medication within appropriate specialty guidelines using the British National formular
- To delegate/refer to other practitioners when this will improve health outcomes or when risk and needs are beyond own competence and scope of practice.
- Accept referrals from other disciplines and provide specialist advice and support to colleagues within the organisation.
Personal & Professional
- Be responsible for maintaining current registration with appropriate professional bodies and perform within any Professional Code and relevant Guidelines for Practice.
- Keep up-to-date with current research and audit findings and base all clinical assessment and management decisions on credible high quality evidence
- Promote integrated working practice between clinical areas and interdisciplinary teams in order to ensure safe patient care.
- Contribute to the development of the Advanced Practitioner role both within the Directorate and Trust wide.
- Maintain own professional portfolio in-line with both local and national competency frameworks.
Service Development
- To actively take part in planning, developing and improving services in order to be responsive to patient needs as well as Directorate, Trust and National Health Service needs
- Participate in implementing national standards and frameworks at a local level within the specialty.
- To be responsible for identifying and addressing his/her own professional, training and development needs, ensuring these training and development needs are aligned to specialty service developments.
Education & Training
- Participate in the initial induction training programme for nursing and medical staff and continue to be involved in their on-going support and training as required.
- Be involved in the bedside, classroom, and directorate teaching of North Bristol NHS Trust staff, as well as utilising additional teaching opportunities such as: national teaching appropriate to the specialty, and teaching/contributing to courses run by the local university, in addition to presenting at national conferences.
- Mentor Advanced practice trainees and medical staff, and supervise other staff working within the clinical area, providing professional support and clinical supervision as required.
- Give guidance and participate in the teaching and assessment of junior, middle grade and senior medical staff.
- Establish links and networks with other professionals in similar roles within the specialty at Local, Regional and National level
- Act as a constant source of clinical and theoretical knowledge for all grades of nursing and medical staff, and patients.
- Be an active member of relevant trust wide professional groups such as the Non-Medical Prescribing Group.
- The post holder will be supported to fulfil any local and national portfolio requirements, adhering to the Four Pillars of Advanced Practice.

- Take part in the senior nurse structure within the clinical area/department, supporting junior members of the team.
- Ensure appropriate representation and participation in clinical/departmental meetings for example: Clinical governance, operational/service development meetings.
- Be aware of the budgetary constraints within the clinical area/department, manage resources appropriately, and encourage others to do likewise.
- Following Trust policy act in an appropriate manner when dealing with incidents and complaints.

- Develop skills and knowledge around research and implementing evidence-based practice into the clinical area.
