To provide and contribute to the development of a specialist memory service diagnostic pathway. This will involve working as part of an integrated, multi-professional team to provide initial specialist assessment, a range of interventions and regular reviews either as clinic-based appointments or in client's homes.

To participate in the Memory Assessment service assessment/screening of referrals.

To provide a specialist and holistic assessment of client's memory, initiate further investigations where needed and present assessments in the multidisciplinary team meeting for diagnosis.

Main duties of the job

The Memory Assessment Service (MAS) is a primary care diagnostic pathway service hosted by the Older Adult Community Mental Health Team for adults with a suspected dementia.

The service offers:

Timely and sensitive diagnosis for people with dementiaTailored information and educationPsychological and medical treatment dependant on needA plan of care and post diagnostic support before return to the care of a GP

To undertake and interpret assessments using agreed assessment tools as part of the diagnostic process for patient referrals made to the Memory assessment service.To communicate/liaise effectively with the team and relevant agencies to ensure that an integrated programme of care is provided during initial assessments until diagnosis.

The ability to speak Welsh is desirable for this post English and/or Welsh speakers are equally welcome to apply.

Person Specification

Qualifications & Knowledge

Essential

  • RMN / RGN/ RNLD
  • Evidence of ongoing professional development to postgraduate diploma level and must be willing to undertake identified local courses / study days and mandatory training
  • A good knowledge of Clinical Governance and ability to put this into practice
  • Commitment to undertake and receive clinical supervision
  • Commitment to undertake training to ensure professional development
  • Excellent Demonstrable knowledge of the MHA 1983 and MCA 2005
  • Understanding of the use and efficacy of antidementia medications


Desirable

  • ENB 998 or equivalent
  • Evidence of specific Dementia related training


Experience

Essential

  • Demonstrable experience of assessing people who experience memory problems
  • Experience of working in a multidisciplinary team
  • Experience of working in a community post


Desirable

  • Experience of contributing to policy and procedure development and implementation
  • Experience of managing and supervising other staff
  • Experience of recruitment and selection of staff
