Band 5 Learning Disability Nurse with the Learning Disability Team based at Erlegh House, Reading and working in the local community. This is a full-time role, 37.5 hours a week.You will be part of a multi-disciplinary team that is passionate about improving the health and lifestyles of people with learning disabilities. You will work with a diverse population in both urban and rural settings providing Community Nursing based on identified needs, to adults who have a learning disability within any community setting.

Main duties of the job
The role will include the following duties
- To manage under supervision a caseload of clients who have been accepted as referrals to the Health Team
- To contribute to comprehensive specialist nursing assessment that takes into account the nature of the learning disability and its implications for care, to include the monitoring and evaluation of programmes of care specific to the persons health needs
- To promote and facilitate access to primary and secondary healthcare for individuals with a learning disability including health checks and health action plans
- To provide specialist advice and therapeutic interventions to parents and carers of people with a learning disability to enable them to support the person with all aspects of their health and behaviour associated with their learning disability
- To work collaboratively with other members of the multidisciplinary team to enable individuals to maximise their health and wellbeing
- To provide and facilitate health education and promotion to include both individual and group work
