A community therapy team across East and North Hertfordshire. We are looking for an Occupational Therapist to join our forward thinking and innovative team to continue to provide excellent rehabilitation to patients within their home environment.

Discharge home to assess, assesses patient within the first 72 hours of discharge from hospital and provide up to 21 days rehabilitation.

The therapist will work alongside physiotherapists, therapy assistants, social workers, community nurses, GPs and carers. Staff meet regularly for professional development and peer support, the post holder will have the opportunity to supervise junior staff and offer placements to students.

This post is full time but applicants who wish to work part time or flexibly will also be considered.

The service runs 8am to 6pm Monday to Friday and 9am to 5pm at weekends. The post holder will be expected to contribute to the weekend rota.

The service covers both East and North Hertfordshire and the post holder may be expected at times to cover across all localities.

You must hold a full valid driving licence and have access to a car to use regularly for business purposes are essential (unless you have a disability as defined by the Equality Act 2010).

Main duties of the job

  • To support the Discharge Home to Assess Team Leads in delivering of high standard therapy service, including service evaluation.
  • To undertake therapeutic assessment of patients, some of whom may have complex and diverse conditions, and delegate tasks as appropriate.
  • Following assessment to provide, develop and implement individual care/treatment plans.
  • To be part of an integrated health and social care team. To provide clinical leadership and support to the multidisciplinary team in providing care to avoid inappropriate admissions to hospital and support hospital discharge.
  • To provide assessments, and treatment, where required for vulnerable patients with potentially complex needs. To support patient-led goal working within the community.

Job responsibilities


To be professional, responsible and accountable to HCT and your professional body for all aspects of own work. This includes the management of patients in your care, and for any treatment plan prescribed.

To maintain a high standard of clinical care for patients under your management and to complete advanced risk assessments independently within the patients own home.

To formulate and deliver an individual Therapy management and treatment programme, based on evidence based practice and expert clinical knowledge. Using advanced clinical assessment, clinical reasoning skills and knowledge of treatment techniques. This may include functional training, behavior modification, patient education, fatigue management/energy conservation.

To evaluate patient progress, reassess and alter treatment programmes if required

To assess and manage clinical risk within own patient caseload, and to report to other professionals as appropriate.

To be responsible for ensuring the effective selection and use of all treatment resources available.

To ensure the implementation of clinical governance frameworks and quality procedures within your team/area of specialty to ensure risk management strategies are in place and apply to all aspects of the work situation and ensure others do likewise.

Patient / Customer Care:

To undertake advanced, comprehensive assessments using investigative and analytical skills.

To assess patient understanding of treatment proposals, gain valid informed consent and work within a legal framework with patients who lack capacity to consent to treatment.

To participate with individuals and groups on health promotion.

To provide information and updates to nursing staff and other members of the team regarding a patients functional, and cognitive abilities

Service Development and Improvement:

To comply with the organisational and service policies and procedures and to be involved in reviewing and updating as appropriate.

To identify improvements in service delivery and clinical practice, including the implementation of relevant aspects of the national service framework and other local/national initiatives.

To be aware of health and safety at work and implement any policies which maintain or improve standards to record and report incidents promptly to senior staff.

Accept Trust policies, ensuring that all quality standards notified by the Trust are understood and met.

Planning and Strategic Management:

To accept clinical responsibility for a designated caseload of patients and to organise this efficiently and effectively with regard to own clinical priorities and use of time.

To act as the single point of contact for the Occupational therapists on a rotational basis within the team whilst managing daily caseload.

To represent the therapy element of the Integrated Service and/or individual patients at appropriate meetings to ensure the delivery of a co-ordinated multidisciplinary service, and integrate physiotherapy intervention into the treatment programme. This may include the discussing of patient care, patient progress and future patient needs.

To decide priorities for own work area, balancing other clinical, non-clinical and professional demands.

To allocate patients visits to rehab assistants or technical instructors as necessary.

To be responsible for organising and planning own caseload to meet service and patient priorities, readjusting plans as demands change

To provide daily capacity figures to ensure effective caseload management.

To chair team meetings as required.

Management and Leadership:

Assist in the performance management for Technical Instructors and Rehabilitation Assistants, using a competency framework, as a line manager

To be responsible for the safe and competent use of treatment prescribed for patients.

To ensure all staff, patients and carers using equipment supplied through HCT are taught, trained and supervised appropriately

To assist with the planning and implementation of induction programmes for new staff and physiotherapy and occupational therapy students.

To be involved in the interviewing process of staff as appropriate.

To supervise the record keeping of student physiotherapists/occupational therapists according to Trust and professional standards to ensure that all legal requirements are met.

To be responsible for teaching student physiotherapists/occupational therapists to graduate level on professional skills and knowledge within core clinical areas.

Communication and Relationship Building:

To act as the patients advocate throughout their care programme and promote patients empowerment and self-management.

To communicate complex, patient related, information effectively to ensure collaborative working with the multidisciplinary team members across all health and social care sectors to achieve a coordinated multidisciplinary service.

To use a range of verbal and non-verbal communication skills to communicate effectively with patients in rehabilitation and treatment programmes, and to ensure an understanding of their condition so that patients are supported in self-management of their condition.

To use skills in persuasion, motivation, empathy, tact, explanation in gaining informed consent with a wide variety of vulnerable patients, often in highly sensitive situations.

To identify barriers to effective communication that will regularly be evident including: cognitive impairment, expressive and receptive communication difficulties, embarrassment, anxiety, pain, fear and visual/hearing impairments to develop and implement strategies to overcome these barriers.

To use communication skills to address language and culture diversity issues.

To maintain accurate and up to date documentation in line with legal requirements and communicate assessment and treatment results to the appropriate disciplines.

To act as an advisor on professional and clinical issues.

To be able to communicate effectively with relatives and carers regarding patient care and progress with the consent of the patient involved.

To facilitate good communication between all members via regular team meetings, verbal and written reporting.

Finance and Resource Management:

Ensure the replenishment of equipment is maintained and demonstrate an awareness of the value of equipment.

Knowledge and Experience:

To develop and implement new treatment techniques using evidence based practice following guidance from the therapy quality lead.

To ensure that own practice and that of staff under supervision meet the required professional standards of occupational therapist practice.

To represent the occupational therapist service and other health professional in own specialty, as requested.

To have degree level or equivalent in occupational therapist and be registered with Health Care Professional Council (HCPC).

To attend/be an active member of any of the following:

in-service training programmes

external courses


training sessions

journal club

staff meetings

To contribute to the preparation and presentation of training sessions and tutorials for in-service training, internal and external courses, and to other professionals and organisations.

To maintain, develop and enhance current knowledge of evidence based practice and developments occurring within specialist field to disseminate information and ensure that best practice is undertaken in clinical speciality.

Information Management:

To use SystmOne to maintain accurate patient records on a daily basis and write reports when required

To achieve and maintain a level of information technology relevant to all areas of work
