This is an excellent opportunity for a highly motivated and enthusiastic Band 5 Community Staff Nurse to join the Adult Community Nursing Team, working flexibly over a seven day period to cover 08:30 to 20:00 hours. Newly qualified are encouraged to apply.
You should have excellent communication skills as this role is key working within the integrated neighbourhood team of co-located health and social care teams.
Main duties of the job
We are part of a dynamic, integrated Health and Social multidisciplinary team delivering high quality care, advice, and support and monitoring to patients, their families and staff within the community settings.
You will be involved with the development of INT working and leading on new initiatives to prevent admissions and facilitate discharge. You will work under the direction and supervision of the team leader to provide care to patients with complex and non-complex needs with the aim of promoting independence and self-management.

Youll be supported by an expert team in an environment where training and development opportunities are actively encouraged in order to meet the clinical needs of the role. We offer a comprehensive induction programme and you will be appraised on a yearly basis with a view to developing your knowledge and skills where appropriate.
Our overseas OSCE programme is currently under review therefore unfortunately we are not in a position to support internationally trained nurses with the NMC registration process. If you are an internationally trained nurse, your application will only be considered if you have completed your OSCE and have had your UK NMC registration confirmed.
