Band 6 Specialist Physiotherapist
Work as part of the Sutton Health and Care Home First service to support avoidable hospital admissions and early supported discharges from the hospital.
Post holders will be working as part of an integrated multidisciplinary team including nurses, occupational therapists, physiotherapists, social workers, rehabilitation assistants, Reablement assistants and administrators as well as the voluntary sector.
The post holder will be based in one of the areas of Home First service initially in the new Richard Bright Reablement Unit but may also have the opportunity to work in the other areas which include rapid response hospital hub (the Emergency Department, Acute Medical Unit and Acute Frailty wards of St. Helier Hospital, rapid response community and intermediate care (bedded and home based care).

Main duties of the job
- To have clinical responsibility for assessment and treatment planning for adults referred as well as active Inreach case finding within the Sutton Health and care (SHC) Home First service
- To work with individual patients, their carers and their families, involving them in decision making about their care and to develop their own health and care plans.
- The post holder will be required to work autonomously and as part of the multidisciplinary care team.
- To supervise junior colleagues and assistants.
- To assist in the overall development and management of physiotherapy and the development of integrated care pathways within (SHC) Home First.
- To collaborate with colleagues across SHC and within the wider health and social care network to optimise patient journey, care, wellbeing and outcomes.
- Participate in flexible working patterns covering extended hours across a seven day service.

Clinical
- To develop a high level of specialist, clinical skill in assessment, treatment planning and implementation of agreed goal orientated intervention with client/carers.
- To assess and treat own workload of patients who may have a highly complex presentation and maintain associated records as an autonomous practitioner.
- To contribute towards single assessment and trusted assessment processes.
- To provide comprehensive intervention to patients, making use of assessment and treatment facilities where appropriate.
- To provide training with patients and their formal and informal carers in the safe use of prescribed equipment and other techniques to optimise a patients functional ability and independence.
- To be responsible for a clinical caseload working with multidisciplinary teams ensuring clients and carers receive optimum level of therapy within available resources.
