We have an exciting opportunity for an Advanced Clinical Practitioner to join our team in Major Trauma, at the Leeds General Infirmary.

You will have completed your Masters in Advanced Practice or we will consider applications from you if you are due to complete in the coming months and are looking to develop into the advanced role within an acute trauma or surgical environment.

The role of advanced practitioner is dynamic and the post-holder will be proactive in pioneering and developing the role further. Successful applicants will be supported in their development as Advanced Practitioners through high quality Clinical Supervision and time for Continuing Professional Development.

Main duties of the job

You will have the opportunity to develop your experience and skills in managing the care of patients with poly-trauma as part of the motivated and committed Multi-disciplinary team. Although experience in this speciality is desirable, we can support you in developing your competencies within this field.

Trauma and Related Services Clinical Service Unit is based at the Leeds General Infirmary.

We are the Regional Centre for Major Trauma and the service opened in 2013 quickly becoming one of the largest and best performing centres in the country.

About us

We are also the Regional acute Vascular Surgery unit, and have a renowned Plastic Reconstructive Service, Trauma Orthopaedic service, Specialist High Observation Ward and Multi-speciality acute Assessment Area.

Although this position will be based within the Major Trauma Department, our growing team of Advanced Practitioners cover Plastic Surgery and the Vascular PoPs team, and we have an ambition to expand further across our range of specialities.

JOB PURPOSE/SUMMARY

The Advanced Clinical Practitioner (ACP) will use specialist knowledge and skills to provide healthcare independently to patients. The ACP is accountable for the total episode of care for patients, participating in early phase or experimental medicine research trials, with undifferentiated and undiagnosed needs.

The post holder will act as a role model, delivering advanced practice and promoting clinical effectiveness in care. He / she will be required to work autonomously in delivering independent assessment, clinical examination, diagnosis, clinical decision making and planning of care and treatment, diagnosis, prescribing, health promotion and safe discharge of patients.

The post holder will work closely with all members of the multidisciplinary team and will play an integral role in the operational/strategic development of the service.

The post holder will promote and facilitate evidence based practice and clinical audit and research within the Trust.

The job holder will work as an autonomous practitioner within the Bexley clinical Research Facility, delivering all aspects of management and care to a wide range of patients with varying needs.

PRINCIPAL DUTIES & AREAS OF RESPONSIBILITY

Clinical practice

Practice as an autonomous professional in a self-directed manner, with confidence, authority and expertise, undertaking comprehensive range of clinical procedures.

Have direct responsibility for patient caseload, ensuring all patients have an accurate plan of care, which reflects the assessment undertaken and incorporate the issues and recommendations made within clinical trial protocols.

Work within the frequently challenging clinical environments with high levels of physical and mental effort

Assess individuals, families and populations holistically using a range of different assessment methods.

Have a health promotion and prevention orientation and comprehensively assess patients for risk factors and early signs of illness.

Interpret and analyse highly complex clinical and non-clinical facts to form accurate diagnoses, prognoses for a wide range of conditions, to recommend / deliver the best course of intervention including the development of comprehensive packages of care.

Initiate pharmacological intervention, using non-medical prescribing or patient group directives and administer medications as prescribed by others in accordance with the Leeds Teaching Hospitals policies

Provide specialist advice and support to patients, their families and other healthcare professionals following assessment, diagnosis and through treatment.

Use advanced clinical skills in the assessment and management of patients within the CRF in collaboration with relevant clinical research teams. This includes obtaining informed consent, history taking, systemic physical examination, requisition and interpretation of appropriates clinical investigations and formulation of clinical management plans, as delegated by CI/PI and within own scope of practice

Plan, manage and complete episodes of care, working in partnership with others and delegating and referring as appropriate to optimise health outcomes and resource use, as well as providing direct support to patients and clients.

Assist PIs and the CRF team to deliver research effectively, safely and escalate appropriately to critical care and outreach services.

Use professional judgement in managing complex and unpredictable care events and capture the learning from these experiences to improve patient care and service delivery.

Organise complex activities or programmes for patients, drawing on an appropriate range of multi-disciplinary and inter-professional resources in practice.

Supervise and where necessary, work in collaboration with nursing, medical and AHP team members, demonstrating expert clinical practice and leadership in learning environments.

To be included on medical rotas as required by clinical role

Maintain a high standard of patient focused care in accordance with Leeds Teaching Hospitals Trust guidelines and policies.

Actively involve service users in providing feedback of their experience of the current service and suggestions for improvements.

Appropriately define the boundaries of own practice.

Improving Quality and Developing Practice

Work within the NMC/HPC Code of Professional Conduct and Guidelines, the Leeds Teaching Hospitals NHS Trust policies and guidelines, COSHH, the Mental Health Act, Good Clinical Practice (GCP) and other instruments of statutory legislation to ensure safe and effective professional working practices for self and others.

Contribute to clinical and research governance processes, including adverse event reporting and the investigation and management of complaints and incidents as appropriate.

Participate in and contribute to any regional and national research to evaluate the effectiveness of care strategies related to the patient experience within the CRF

Alert appropriate individuals to gaps in evidence and/or practice knowledge and support and conduct research that is likely to enhance practice.

Use financial acumen in patient/client, team, organisation and system level decision making and demonstrate appropriate strategies to enhance quality, productivity and value.

Qualifications

Essential

• Registered with relevant professional body
• Degree-level registration qualification or equivalent post-registration award MSc in Advanced Practice (or PG Diploma in advanced practice for those with other Masters qualification)
• Life Support qualification
• Relevant teaching qualification.
