- The post holder will provide specialist nursing advice and support to patients with Heart Failure attending Cardiac Rehabilitation, and facilitate high quality, individualised care to patients/ clients and relatives/carers, in collaboration with the multi-disciplinary cardiac rehabilitation / heart failure team in line with local and national targets. This will involve dealing with complex situations and having to take appropriate action in a variety of settings.
- To be flexible in the approach to the clinical area and to provide cover for the Cardiac Rehabilitation service acting as professional role model providing direct and indirect clinical care, teaching and research.
- To develop the Cardiac Rehabilitation Nurse (Heart Failure) role in accordance with local and national policy and ensure the needs of patients and their families are central to service improvement initiatives.
To deliver a home-based cardiac rehabilitation programme to patients and carers via the Reach Heart Failure Manual Programme.
Communicate and explain the programme to patients and their carers in an individualised, compassionate, and caring manner.
Motivate and encourage patients though their cardiac rehabilitation journey.
To work as a specialist practitioner to support the effective delivery of a nursing service within thecardiac prevention and rehabilitation service.
Main duties of the job
- To lead by example
- To work towards additional qualifications that supports the development of specialist knowledge and expertise.
- To contribute to the provision of specialist nursing advice and support to patients, their families and other healthcare professionals following diagnosis and through all stages of treatment.
- To provide a high standard of evidence based nursing care, using an in-depth assessment of health related and nursing needs of patients, their families and other carers to identify and initiate appropriate interventions for effective care.
- To ensure all patients have an accurate plan of care, which reflects the assessment undertaken and incorporates the issues and recommendations made ensuring clear documentation in the patient's records (hand held and electronic records)
- To provide and manage an out-patient case load for patients discharged with relevant cardiac conditions, providing on-going advice and support and facilitating entry to the formal cardiac rehabilitation programme.
- To work with key staff members to maintain nurse led pre-assessment/assessment and out-patient follow-up services. Providing detailed baseline patient assessment and education and support to patients and their families.
