The Crisis and Home Treatment Service covers the geographical area of Telford and Wrekin and based in Hall Court, Telford to deliver culturally sensitive community based interventions. The team plans, implements and reviews a period of home treatment to safely avoid hospital admission based on service user need. This also involves undertaking assessments and providing home treatment to facilitate discharge from in patient care through close working with Community Mental health Teams and inpatient services.

To provide clinical leadership and clinical expertise in the Crisis/ Home treatment Service in Telford, contributing to the delivery of effective assessments and person centred interventions that support Crisis Resolution Home Treatment Service delivery. The post holder, working as part of a multidisciplinary team, will focus on supporting the delivery of Home treatment to support recovery.

- The post-holder is responsible for working collaboratively with other services to provide seamless care for service-users on defined care-pathways and within defined care clusters

Qualifications
Essential
- Possesses first level qualification Part 3 or 13 of the NMC Register and maintains current registration OR First Degree Level qualification/DIPSW Practice

- Current Approved Mental Health Professional status or commitment to undergo appropriate training to achieve and maintain AMHP status
Experience
Essential
- Experience (equivalent to post-graduate level of practice) of working effectively, delivering/managing care and associated risks in mental health settings

- Experience of working effectively in a community mental health setting
