An exciting opportunity has arisen for an experienced occupational therapist, physiotherapist, registered general nurse or registered mental health nurse, with a passion for the care of people with both dementia and delirium and their families/ carers. We are looking for a dynamic and self- motivated individual with experience in dementia and delirium care. This post requires autonomous working across University Hospitals Southampton NHS Foundation Trust (UHS) supporting staff to deliver all aspects of the UHS dementia and delirium strategies. The Dementia and Delirium Teamwork across all ages and all areas of the hospital to improve the knowledge and skills of colleagues and the patient and carer experience.

Job responsibilities

An example of some of the duties this diverse role would include:

Promoting person centred care Develop and support wards in producing care plans Work alongside staff to identify triggers and solutions for people with dementia/ delirium that are distressed Complete MCA and DOLS assessments as appropriate or support other disciplines in the process Support the dementia champions education programme Deliver teaching and training sessions in a formal and informal capacity Provide psychosocial interventions supported by signposting to family/carers hub Engage with patients in activities, communicate benefits and outcomes to ward staff Working alongside the Older Peoples Mental Health liaison team

  • Allied Health Professional, OT or Physiotherapist, registered general nurse or registered Mental Health Nurse.
  • Mentorship qualification (or evidence of meeting the outcomes of the NMC or HCPC standardsEducated to or working towards Degree level.


Desirable

  • Specialist qualification.
  • Recognised teaching qualification


Previous or relevant experience necessary

Essential

  • Extensive post registration experience, including considerable experience at higher Band 5 within relevant speciality
  • Recent experience of working with people with dementia and their families
  • Knowledge of current issues within acute Health Service.
  • Understanding of personal accountability.
  • Logical and consistent work and career pattern.
  • Proven management experience.


Desirable

  • Proven self management experience
