This is an exciting opportunity for an experienced Nurse to work as a Band 6 District Nurse/Senior Community Nurse as part of our friendly Community nursing team.
Up to 30 hours per week
The role requires positivity and excellent communication, leadership, and caseload management abilities. You will need to demonstrate integrity and initiative, sound clinical knowledge with evidenced based practice and the ability to manage a team, approaching your work in a coordinated way.
We are integrated within the primary care networks, adult social care, therapists, urgent & intermediate care services alongside our specialist services informing our ageing well MDTs which facilitates shaping our future to deliver great quality care for housebound individuals.

Main duties of the job
To ensure provision of a nursing assessments/high quality/safe/ evidence-based care to patients/carers with the aim of maintaining patients in their own homes
To work in partnership with patients/carers/members of the multi-disciplinary team to offer support/advice/promote informed choice, maximising independence/optimising quality of life
To be an autonomous practitioner responsible for the assessment and treatment of patients with complex health/social care needs
To provide developmental management to all staff within the team supporting the Band 7
To work in this adaptable/evolving role, focusing on safe/effective admission avoidance, supporting timely discharge from hospitals/other care settings. Implementing personalised support plans/collaborating with the whole community resource to facilitate ongoing care management
To be a Community Sister your role is vital in terms of coordinating peoples care/delegating to your team. Develop knowledge of government policy/clinical procedures
You will be required to be an autonomous practitioner/a role model/to provide holistic patient centred care. To champion for patients, whilst demonstrating highly motivated leadership skills with ability to influence/inspire others. Teach/mentor your team utilising your transferable skills
We offer training in line with the required competencies for you to build on your past knowledge/experience/skills. You will be expected to undertake continued learning with accessibility to accredited courses
