To provide a comprehensive and integrated specialist nursing service for people living with diabetes both in the Hospital and in the Community. The post holder will join other colleagues providing a service for adult people with diabetes. They will act as an additional resource for nursing colleagues and other health workers offering expert care, advice, teaching and support and building up communication links as needed. Flexibility is essential in this post.

Main duties of the job

Main duties include providing an inpatient diabetes service. Assessing and reviewing acutely unwell patients to enable they have a positive patient journey.

Ensuring that they are receiving high quality diabetic care as well as ensuring that their medications are appropriate and patient centred to achieve good outcomes and reduce long term effects of diabetes.

Supporting ward staff to adopt care plans specific to the individual and providing ongoing education.

Supporting patients with the available technology eg CGM, Flash glucose monitoring and insulin pumps.

Empowering patients to take control of their diabetes and promote self management.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

Job description

Job responsibilities

To develop the awareness of people with diabetes and where appropriate their relatives/carers into the most efficient use of their diabetic equipment.

To develop the awareness of people with diabetes and relatives/carers about factors contributing to the promotion of good health/improvement in the management of diabetes, to ensure all possible assistance is identified to provide an acceptable independent quality of life for the patient involving pre and post discharge visits.

To act as a resource by providing teaching sessions for Hospital/Community based Health Care Professionals as required

To organise and have an active role in the planning, delivery and education of patients, their families and carers.

To provide insulin education workshops for patients, families and carers, which are continuously evaluated for effectiveness through audit and developed to need.

Provide people with diabetes and their families with verbal and written information/advice about their illness and treatment and management of newly diagnosed or long standing diabetes.

To assist in health education and to provide advice on any possible lifestyle changes that may be required, i.e. diet, exercise, smoking, foot care, pregnancy
