An exciting opportunity has arisen for a Band 6 Community Practitioner to work into the Step up Hub Team based at Palmer Community Hospital, Jarrow. We are a community based mental health rehabilitation team working with service users experiencing Psychosis, delivering evidence based interventions to support rehabilitation within a community setting.
The Step up Hub service covers the population of South Tyneside and Sunderland, working with people who require intensive support to aid recovery in the community. We are a multi disciplinary team who work collaboratively with service users taking a holistic approach to care planning and delivery.
The successful candidate will work as part of the Step up Hub team and will have an interest in the mental health rehabilitation pathway . It would be extremely beneficial to have experience of working with service users with a Psychosis however, there are excellent opportunity's for training in the Pathway.
Working within an experienced and highly motivated clinical team you will be part of a service engaged in ensuring the best possible practice in contemporary mental health care implementing NICE guidelines. Your job role will be to provide assessment and treatment to service users, their families and carers, whilst working collaboratively with other professionals and agencies.
Main duties of the job
To be responsible for the assessment and management of casework of service users who have substantial and complex needs.
Provide specialist clinical advice to service users, carers, staff, statutory agencies, 3rd sector partner agencies and the public.
In collaboration with the service users, develop recovery focused care plans, ensuring completion of regular reviews to monitor progress and adapt care plan as necessary.
To consider carer needs and ensure carers and family are involved in care planning where appropriate consent is in place.
To deliver evidenced based clinical and psychologically based Interventions.
The post holder will work as a member of the Multi-disciplinary Team ensuring that high quality individualised care is delivered which ensures safety and a commitment to promoting recovery and wellbeing and maximising independence.
Coordinate treatment planning & interventions, reviews and discharge planning.
Provide clinical supervision to staff as delegated by the Pathway Manager.
To provide compassionate care that is based on empathy, kindness, respect and dignity.
To incorporate a client centred philosophy into nursing practice and client centred collaborative care.
Work flexibly to meet the needs of the Service Users and the organisation.
