An exciting opportunity has arisen for a passionate and committed qualified practitioner to join Moss House community mental health team. This post is suitable for a qualified mental health nurse, occupational therapist or social worker. Experience in adult community mental health is desirable.
The practitioner will operate as part of an innovative, forward thinking team, attending weekly multidisciplinary meetings, participating initial assessments, follow ups and reviews of service users as part of their own caseload.
The practitioner will work in a community setting, meeting service user mental health needs within service user's homes, the CMHT base and in the wider community. They will complete care plans, risk assessments, use outcome measures and will deliver psychosocial interventions appropriate to service user's presentations.
In return they will be supported by the CMHT team, both by practitioner colleagues and team leaders.
Main duties of the job
Assess, care plan, deliver interventions and evaluate effectiveness of these interventions for service users within a CMHT.
Liaise with other relevant agencies.
Work within an MDT to support colleagues and provide high standard of care.
