Do you want to be part of the revolution in patient Supported Self Management that will provide a dramatically different approach to follow up care? We are transforming the way we deliver post treatment Cancer Care at BHRUT. Our vision is to support our cancer survivors to live well beyond their treatment, close to home with just the right amount of contact with us that feels right to them. We are looking for a registered nurse to join our Personalised Stratified Cancer Follow Up (PSFU) team and help us realise this vision.

The successful applicant will ramp up the delivery of PSFU across several cancer services. The post holder will work collaboratively with the Lead Cancer Nurse Cancer nurse, Personalised Cancer Care Team, and clinical teams to ensure that the stratified pathways are delivered across multiple cancer sites. They will ensure that patients receive personalised, holistic supportive care through their pathway. The post holder will have an opportunity to support with Trust Cancer Wellbeing events, education, health promotion and will develop links with wider community services. You will be supported in your development by the ACCEND programme for cancer professionals, access to Cancer Practice development nurses as well as internal and external education, coaching and mentoring.

Main duties of the job

The post holder will be responsible for all aspects of the Remote Monitoring Pathway for patients. They will act as the expert and superuser for the Remote Monitoring System (RMS) module of the Somerset Cancer Registry. This will include writing and developing protocols for remote monitoring and supported self management in line with regional and national best practice, enrolment of patients, monitoring of patients on a stratified follow up pathway, escalation of concerns where necessary.

The post holder will be the lead in promoting the uptake of RMS and educating others in using the system to meet expected KPI's and targets for RMS and Personal Stratified Follow Up. They will cultivate strong links with appropriate MDTS's to identify suitable patients,

We know we have more work to do to improve waiting times for urgent and emergency care, and our performance against the four-hour emergency access standard remains challenged, in comparison to many other London trusts.

We are particularly proud of our regional Neurosciences Centre Radiotherapy Centre Hyper Acute Stroke Unit and dedicated breast care service at King George Hospital. We're also pleased to be part of the NEL Cancer Alliance.
