An exciting opportunity has arisen to work within the Adult mental health community services at a time of innovation and service development. This role will be involved in a project that is developing a bridge between the newly created Rehab model and the developing community Core team offer and will be vital in enabling our community teams to adapt the existing services within the new transformation process and rehab pathway. This process is in response to the NHS 5-year plan to develop mental health services around the Primary Care Networks in Barnet and provides new opportunities which include developing partnership working with the voluntary sector, working more closely with GP practices and developing more innovative, community based and holistic ways of working focused on a rehab and recovery model.
The post holder will also be part of a daily Mental Health MDT referral meeting where all new referrals will be screened and allocated to the relevant Mental health practitioner/named worker, for a Dialog+ assessment and care plan - depending on presenting needs.

Main duties of the job
- To Be part of a daily Mental Health MDT referral meeting where all new referrals will be screened and allocated to the relevant Mental health practitioner/named worker.
- To undertake Full DIALOG +
- Carry a clinical caseload of service users, supporting initial assessments within 4 weeks for a DIALOG+ care plan in line with the national performance indicator (KPI) and working closely with colleagues within social care, rehab and core team.
- To ensure that people referred for intervention are quickly and efficiently able to access the most appropriate treatment for their mental health difficulties
- Deliver 1:1 support/interventions based on an individual Dialog+ care plan, with recovery focused outcomes for service users
- To Work closely with the core teams to enable both the prevention of further access to Mental Health services as well as participate in the delivery of the discharge framework, supporting move on from service - supporting the continuity of care for a person recovery journey through services and within their own home and environment.
- To be proactive in ensuring service users receive the range of interventions they need in line with their care plan
- To participate fully in caseload supervision and management, to ensure that care plans are matched to need and service users are able to 'flow' through pathways of care.

Education and Qualifications
Essential
- Relevant Degree of qualification in Mental Health and registration to the relevant professional body
- A registered qualification in Mental Health such as Mental Health Nursing or Psychosocial, AHP or Social Care.
Desirable
- Relevant qualification in Psychosocial Interventions with people with severe and enduring mental health problems
Experience
Essential
- Understanding of statutory policy relating to mental health care delivery including NICE guidance and the Mental Health Act
Desirable
- Working with people with complex mental health problems.
- Understanding recovery in mental health settings
- Experience as a Team Leader/Manager or Deputy Team Leader/Manager.
