Ramsgate House Community Mental Health Teams are looking to recruit a motivated and enthusiastic Senior Practitioner for Co-Occurring Conditions (Dual Diagnosis) to join a successful, friendly and supportive teams. It would be desirable that the applicant has knowledge and experience working with service users with substance misuse issues.

You would be part of a large and experienced senior team within the CMHT. Also, monthly team building exercises, reflective practice groups, and mindfulness.

We are a committed integrated service that strives to provide quality recovery-based support to people with severe and enduring mental health related difficulties within the Salford area.

Main duties of the job

Provide clinical leadership, supervision, support, and consultation to MDT staff in the Community Mental Health Team, in line with co-occurring conditions clinical pathways and the standard operating procedure

Undertake liaison and develop effective working relationships with GMMH and statutory and non-statutory partners

To maintain own clinical and professional competence.

To participate in the duty rota for CMHT.

There is an expectation that the post holder will participate in out- of-hours management on call rotas in providing management. To participate in the service bronze on call rota.

Deputise for the Team Manager in his/her absence and take on any delegated duties as required.

Manage and evaluate a complex caseload of service users with co-occurring conditions in line with relevant clinical pathway as outlined within the operational procedure and will include: -
  • Assessment
  • Formulation
  • Therapeutic intervention as required
  • Signposting
  • Close working with all colleagues
  • To be responsible for a complex caseload

To be an active link and collaborator with substance misuse services in Salford

Person Specification

Qualifications

Essential

  • Core Professional qualifications
  • Registration with appropriate professional body


Desirable

  • evidence of post-registration study such as a Master's degree in a subject relevant to the specialty of health and social care, leadership or specialist practice appropriate to specialty of CMHT or relevant discipline.


Experience

Essential

  • Evidence of applied post grad theoretical knowledge in specialist area of clinical practice
  • Experience of working in a community setting
  • Experience of working with service users with co-occurring conditions
  • Detailed knowledge and experience of risk assessment and management within community settings.
