We are recruiting a Living Donor Co-ordinator to be responsible for the for the co-ordination and organisation of individual treatment programmes throughout the living donor pathway.

This post is a key role in the multidisciplinary team. The successful candidate will be required to undertake 1:6 on call for local and national donors.

***Preference will be given to internal Trust staff, as well as 'Priority' and 'At Risk' status including NHS At Risk staff throughout Devon who are able to display recent relevant experience as dictated by the Person Specification.***

Main duties of the job

Assess, plan, implement and evaluate expert nursing care and provide Specialist nursing support to potential live donors and recipients, as well as their carers within the South West Peninsula. The post holder will have continuing responsibility for the co-ordination and organisation of individual treatment programmes throughout the living donor pathway and work to develop communication across divisional, regional, national, occasional international and organisational boundaries. This post is a key role in the multidisciplinary team. Rotate around all areas within transplantation. Undertake 1:6 on call for local and national donors.

The Renal Transplant Team are seeking an enthusiastic individual for the post of Living Donor Coordinator. This is a fixed/secondment position to cover maternity leave for 37:5 hours per week available at Band 7 .

The role will include a 1:6 (pro-rata) on call service for local and national kidney offers.

Job description

Job responsibilities

  • Responsible for his/her own caseload of patients entering the live donor programmed whilst working as part of the Multidisciplinary team
  • To provide information and support to people coming forward as potential living kidney donors identifying their psychological, physiological, emotional, spiritual and cultural needs
  • To conduct initial health screening of potential donors and facilitate recruitment and triage of referrals
  • To provide education about the donation process including compatibility testing, investigations/screening, the surgical procedure, the legal approval and aftercare involved to enable the donor to make an informed choic
  • To undertake social and physical assessment of the potential donor maintaining appropriate documentation on vitaldata and in the hospital records. To request relevant investigations following the living donor pathway, coordinating and facilitating the one stop live donor clinic to streamline the work up process
  • To undertake venepuncture, cannulation, weight, blood pressure, urinalysis documenting all information on vitaldata
  • Liaising with the H& I lab to test samples of DNA for tissue typing between donors and recipients. Discussing any complex matching with the wider MDT
  • To triage referrals coming in from the different referral centres and continuing the assessment process. Allocating a local nephrologist to these potential donors and arranging for a desktop notes review
  • To present potential donors to the multidisciplinary team for review, presenting their medical history, relevant results and details of the assessment. To follow up on any further information required and/or referral to other specialties
  • Responsible for communicating highly sensitive and emotive condition related information to patients and relatives. e.g. unsuitability to donate. Informing potential donors of the outcomes of multidisciplinary discussions
  • Coordinates timely referral to surgical colleagues for assessment. Including facilitation of pre-operative assessments and anaesthetic reviews
  • Refers to the human tissue authority to ensure that appropriate legal approval is gained for living donor transplants to take place. Follows guidance set by the human tissue authority
  • Liaises with surgeons and theatre to ensure theatre dates are planned in a timely manner. Liaises with donors and recipients to book in these dates for the potential transplant to go ahead.

Knowledge & Experience

Essential

  • Demonstrable renal transplant experience essential.
  • Demonstrable continuous personal, professional, and academic development.


Desirable

  • Cannulation.


Qualifications

Essential

  • Registered Nurse.
  • Post reg. specialist qualification (renal course).


Desirable

  • Counselling course.


Aptitude & Abilities

Essential

  • Demonstrable ability to prioritise workload, achieving a balance between clinical and other aspects of the role.
  • Deputise for the Nurse consultant.
