Following the successful approval of the Additional Roles Reimbursement Scheme (ARRS) funding, Hertfordshire Partnership University NHS Foundation Trust (HPFT) Children and Young Peoples Mental Health Services and Hertfordshire Primary Care Networks (PCNs) are pleased to announce this unique and exciting opportunity to develop and implement a new way of working within local communities which puts Children and Young People and their families at the heart and centre of what we do as part of a wider transformation and expansion of community based mental health services, set out in the NHS Long Term Plan.

We are looking for a passionate leading Mental Health Practitioner Band 7 to join us in our CYP Mental Health Primary Care services (CYP MHPCS). This is a full-time permanent position and driving is essential as you will be required to cover various practices within the PCN sites.

Our CYP MHPCS model is at the forefront of innovation to offer a real chance to make a difference to our children and young people and their families via their local GP practice to increase capacity at the earliest point and making support available where and when it is most needed.

The role would suit someone who is confident working autonomously using their own initiative but also being part of a team. We are looking for professionals who work dynamically in a fast-paced environment, managing smaller caseloads with a quick turn-over but offering a service of the highest quality.

An added advantage is the option to work more flexibly and closer to your own home as there are various posts in all quadrants in Hertfordshire.

Job description

Job responsibilities

This is an early intervention service aimed at CYPs with mild to moderated mental health and emotional difficulties. The main duties will include triage, consultation, advice and support and evidence based low-intensity interventions. A major part of the role will be consultation and signposting to a wider range of community mental health services.

As a Senior Mental Health Practitioner you will be embedded within the PCNs multi-disciplinary team as the first contact for Children and Young People presenting with mental health and emotional difficulties. You will work alongside other PCN professionals including GPs, nurses, and other ARRS practitioners to ensure a holistic approach by providing early and timely assessments to ensure the right information, advice and support is provided at the right time.

This integral innovative way of working will seek to eliminate current barriers to accessing services whilst meeting the needs of our CYP, carers and families at the earliest point to avoid deterioration in presentations and improve population health, experiences and outcomes close to their own homes.
  • A key co-ordinating role at the heart of the GP practices and are a fully integrated member of the multi-disciplinary team with regards to response and management pathways.
  • Work with CYP up to the age of 18 whose emotional and mental health needs can be best met within primary care services and whose difficulties are best understood within a biopsychosocial
  • Act as a first point of contact within the PCN and triage referrals.
  • Be able to work dynamically and autonomously to provide a combined triage, consultation, advice and liaison function in accordance with agreed protocols
  • Support CYP and their families who present to their GP with identified or suspected emotional and mental health issues such as anxiety, low mood, eating difficulties.
  • Develop a holistic personalised care and support plan with the young person and their families. Responsive to prevent difficulties escalating and integrated working to ensure the child/young person is receiving the right care at the right point.
  • Be able to identify and intervene to meet presenting needs by providing extra time, capacity and expertise to make sure that the appropriate support is made available to CYP and their families to ensure that their needs are addressed timely and effectively.
  • Work closely with PCN colleagues and other community care-based services to ensure CYP and their families receive the care that is responsive to their needs and designed in co-operation with the young person and their family.
  • Promote and deliver low level early intervention and treatment which may include brief psychological interventions as well as discharge appropriately when completed.
  • Facilitate onward access to other community care-based services, or third sector services that are integrated within the CYP mental health Getting Help pathways.
  • May supervise junior staff members and coordinate the day-to-day functioning and delivery of the clinical They will also receive appropriate clinical and managerial supervision from their Line Manager or appropriately identified lead.
  • Contribute to delivering and co-developing the primary care mental health service model. This includes participating and contributing to the development of care pathways for the provision of integrated physical and mental health care and also offering consultation and training to primary care team.


Person Specification

Qualifications

Essential

  • Current professional registration: Registered Mental Health Nurse, Social Worker, Occupational Therapist or equivalent
  • Evidence of relevant post registration training and development in particular motivational interviewing, brief solution focussed interventions, cognitive and behavioural approaches and psychosocial interventions


Desirable

  • Independent and supplementary Prescriber
  • Recognised Post Graduate clinical or leadership qualification relevant to the role
  • Mentor and mentorship training


Experience

Essential

  • Extensive post registration experience in working with people with mental health needs.
  • Community experience in mental health services for adults and older age adults with severe / significant mental illness
  • Experience of providing a range of evidence based interventions to people with a variety of mental health problems and serious mental illness
  • Considerable experience in the assessment and management of clinical risk.
  • Experience in physical health assessment for people with mental health needs.
  • Experience of effective collaborative working in partnership to deliver holistic health and social care outcomes.
  • Evidence of Interdisciplinary / multi-agency working experience including primary care and community/ voluntary sector.
  • Experience of leadership and development of junior colleagues.


Desirable

  • Proficient in clinical audits and research.
  • Experience of working in primary and secondary mental health services.
  • Experience in the planning and delivery of training and practice developments to staff.
  • Experience in service evaluation and improvements.
  • Experience working in a CAMHS and/or OPMH service
