We are recruiting to the role of Integrated Therapy Co-ordinator within our Community Health and Integrated Care Division to work alongside our Advanced Clinical Practitioner in Community Rehabilitation. The role will provide support to Quality Improvement projects and the implementation of the Rehabilitation Strategy for Stroke. It's an exciting journey to be a part of!
We are looking for a passionate person who works well in a team environment, is highly motivated and committed, enjoys autonomy within their role and can support the project aims using their initiative and knowhow to move things forward. This role will require frequent travel across all ESHT sites and may require you to attend meetings with our Advanced Clinical Practitioner outside of the organisation.
The ESHT Rehabilitation strategy is focused on improving the quality and performance of our acute and community services to deliver high quality rehabilitation across East Sussex. This is a dynamic and changing environment, and the strategy is working to support our staff, teams and the organisation as a whole to make meaningful changes that bring about improvements in rehabilitation for the benefit of our population living in East Sussex.
Main duties of the job
To work within SQuIRE project team to provide a flexible range of integrated, therapeutic services for the local stroke population to identify and review individual clients' needs, ensuring that they are receiving an effective, equitable and personalised service. To coordinate timely client care by supporting and encouraging integrated working across multiple services and organisations, engaging with a broad range of health and social care providers. As a member of the project leadership team to contribute towards and support the development of quality indicators and metrics to evaluate the impact of the role in supporting service users with complex stroke.

- Educated to level 5 with a Foundation Degree in Health and Social Care OR other relevant level 5 degree/diploma/certificate/NVQ
- Recognised IT qualification or evidence of equivalent IT training
- Risk assessment training /experience
- Proven CPD in a relevant health/social care context to diploma equivalent

- Relevant degree and/or clinical professional experience
