An opportunity has arisen for a Community Dietitian to lead our community team, due to the resignation of the current post holder. The team, consisting of Dietitians, Assistant Dietitian Practitioners and an Administrative Assistant are based Chesterfield Royal Hospital alongside our acute adult and paediatric dietetic teams and provide services across North Derbyshire.

This post provides a varied caseload across a variety of locations and we are able to support flexible working and working from home.

Main duties of the job

  • To provide nutrition and dietetic services to adults and their carers across the North Derbyshire community.
  • To take lead responsibility for the Community Nutrition and Dietetic Service development for the North Derbyshire health community.
  • To operationally manage the Community Nutrition and Dietetic clinical service provided at Chesterfield Royal Hospital NHS Foundation NHS Trust.
  • To develop and contribute to service developments for Community Dietetics with the support of Head of Nutrition and Dietetics.

    Job description

    Job responsibilities

    Clinical.

    1.

    To provide a client centred service to adults at various locations according to need.

    2.

    To participate in the team approach to the care of adults who are referred to the service.

    3.

    To liaise with medical, nursing, pharmacy, catering and other staff within the trust and the wider health community to ensure the nutritional needs of adults referred to the service can be met.

    4.

    To accurately collect relevant activity data required for service monitoring, management and evaluation.

    5.

    To develop evidence based dietary information in conjunction with other members of the Nutrition and Dietetic team.
    6.
    To provide cover for dietitians as appropriate.

    Operational Management.

    1.

    To deliver with the Head of Nutrition and Dietetic Services the active management of all aspects of the Community Nutrition and Dietetic service.
    2.
    To manage Nutrition and Dietetic personnel working within the Community Dietetic Service to ensure adequate cover arrangements are in place, Trust targets are met, and patients are seen according to clinical need.

    3.

    To identify and implement service developments within the Community Dietetic Team to ensure agreed service objectives are met and to support the innovation and transformation agenda.

    To deputise for the Head of Nutrition and Dietetic Service as required.

    5.

    To promote recruitment and retention strategies that ensure appropriate skills are available to meet the needs of service users.

    Clinical Governance.

    1.

    To be actively involved in the implementation and development of Quality Governance.

    2.

    To advise the Head of Nutrition and Dietetic Service on professional issues relating to community dietetics.

    3.

    To work with the Head of Nutrition and Dietetics to ensure the nutrition and dietetic training needs of nursing, medical and other staff working within community are met within the Trust and wider health community.
    4.
    To contribute with other members of the nutrition and dietetic team to the identification, development and implementation of standards of dietetic practice.

    4.

    To represent the Nutrition and Dietetic Service at meetings and forums within the hospital and the health community.
    5.
    To participate in the staff appraisal and clinical supervision process.

    6.

    To actively participate in clinical audit.

    7.

    To demonstrate a commitment to CPD by maintaining an up-to-date professional portfolio.

    Person Specification

    Qualifications

    Essential

    • HCPC registration
    • Postgraduate dietetic training


    Experience

    Essential

    • Significant dietetic experience at Band 6
    • Experience of working with adults in an inpatient and outpatient setting
    • Experience of working with individuals who are enterally fed
    • Involvement in practice placements
    • Teaching experience


    Skills and Knowledge

    Essential

    • Excellent written and oral communication skills
    • Ability to work collaboratively with others
    • Ability to work as part of a team
    • Knowledge in the field of community dietetics
    • oEvidence of leadership and team building skills
