An exciting opportunity has arisen to work as a Band 6 Physiotherapist, working within an intermediate care multi-professional team based at the new Sister Winifred Laver Promoting Independence Centre, whilst potentially also providing some support to patients within Gateshead Community.

The post holder will work effectively within a multi-disciplinary/agency team attending to patients identified as requiring intervention to prevent a deterioration in their health and wellbeing and to promote and encourage rehabilitation. In addition, there will be planned activity to provide hands on care and holistic assessment to prevent avoidable admission to hospital, and/or long-term care and to maximise patient independence/promote rehabilitation to allow the patient to return to their home environment.

This post holder will be required to ensure the fundamentals of care are provided at the point of contact and will ensure delivery of a range of appropriate interventions to be able to meet the needs of the patients' health and wellbeing, within the community setting.

This post will contribute to the safe delivery of effective rehabilitation working flexibly over 7 days per week.

Main duties of the job

• The post holder will provide vital support and integrated seamless services within community settings, working in partnership with local authorities, other health care providers, GP practices and the acute hospital trust
• The post holder will work within the multidisciplinary team also liaising with multiple stakeholders in order to ensure that the day to day co-ordination and delivery of service is in a timely and professional manner.
• The post holder will play a key role in delivering a range of therapy interventions which enable the patient to receive the optimum care health outcomes. There will be a need to understand and undertake administration and co-ordination of tasks within the team as and when required.
