This post provides a unique opportunity to join the Pharmacy team at the Royal Devon University Healthcare NHS Foundation Trust and enhance our ability to provide a Clinical Pharmacy Service to Community Hospital Adult Inpatients as well as a Medicines Optimisation Service to patients in their own homes.
To provide a high-quality Specialist Clinical Pharmacy Service to patients in their own home and within Community Hospitals as part of an integrated team.
Develop relationships with other Pharmacy professionals and members of the Multi-disciplinary team to support integration across Health and Social Care including Primary Care, Community Pharmacy, Secondary Care, Acute Hospital at home and Mental Health.
Ensure safe, effective and economic medicines use to strive for best outcomes for patients.
To be responsible for linking with the wider Health and Social Care Teams to optimise medication regimes and pharmaceutical care.
Support Pharmacy Technicians with Clinical Prioritisation on a day to day basis.
To provide advice, education, training and development for patients, other Pharmacy staff and members of the Multi-disciplinary team.

We are looking for an experienced, innovative and flexible Pharmacist to join our team and enhance our ability to provide Pharmaceutical support for patients receiving care in Eastern Devon Community Services.
This role is ideal for a Pharmacist with previous experience of working in Hospital Pharmacy but is also suitable for candidates who may currently be working in another Pharmacy Sector as full training and support is available. Applications will also be considered from candidates who may wish to combine this role as part of a portfolio career.
You will be an excellent communicator interacting with members of the Pharmacy team, other Healthcare professionals, Patients and Carers. Applicants should be passionate about patient care and strive to ensure patients receive the best possible outcomes from prescribed medicines.
Working knowledge of discharge processes and an understanding of medicines issues across interfaces of care would be an advantage. Candidates will be able to develop their Clinical skills and experience in this role but should have a sound Clinical Knowledge base, ability to problem solve, make decisions and be able to prioritise workload effectively.
Following a period of induction, you will be expected to provide supervision, training and support to junior staff and to support the wider community services pharmacy team with the delivery of training to other members of the multidisciplinary team on prescribing and medicines issues.
In return you will be a member of the established Community Services Pharmacy team who will support you with personal development with appropriate training and opportunities to further develop your skills.
You should be highly self-motivated with an ability to plan and work flexibly, independently and as part of a wider team. Good IT skills are essential for this role.
This post is based in Tiverton but you should be able to travel to various locations for meetings and patient visits as required.
