The Complex Care at Home Service (CC@H) is looking to recruit an experienced physiotherapist to join the team. Ideally, we are looking for a therapist with experience of working in a community setting with a knowledge of case management. If you are an experienced Band 5 physiotherapist with community experience who would like to develop into this area of practice, we would be able to offer you a development programme leading to a Band 6 role, over a 12-month period,
CC@H provides proactive preventative case management to a mainly frail older cohort of patients, but not exclusively. We accept people into our service living with complex health and care needs living with frailty.
The team consists of matrons who are nurses and allied health professionals, therapists, including registered dietitian, wellbeing coordinators, care navigators from adult social care, frailty housing officer and clinical support and advice from consultant geriatricians.
Main duties of the job
Our team love coming to work in a role that allows them to use a health coaching approach to support a person achieve their goals. We always try to use Health Coaching and Motivational Interviewing skills and utilise outcome measures that evidence their effectiveness in improving a person's health and wellbeing.
The CC@H service is a truly multi-disciplinary team who support people living with complex health conditions and frailty to remain as independent as possible in their choice of residence. The role is rewarding and challenging but never boring. You will be supported by an amazing team of clinicians, health and wellbeing coordinators and social care colleagues.
You will be visiting people in their own homes and supporting them to live well and improve their resilience, with a focus on what matters to them and their concerns.
