We are looking for an enthusiastic and dynamic Dietitian to join our well respected community dietetic team in Pembrokeshire to provide a specialised clinical dietetic service for the acute / urgent response team. We welcome applicants that would like to consider flexible and part time working (minimum 4 days per week).

This is an exciting post and offers an ideal opportunity for a Dietitian to fully embed themselves as part of a wider MDT, supporting embedding nutrition/hydration as part of models of care .The post-holder will be based in South Pembrokeshire hospital alongside the integrated care team.

Main duties of the job

Acting clinical dietetic lead for the rapid response team / community integrated care team, working collaboratively with proximal community multidisciplinary services to strengthen communication and clinical delivery across community settings.

Autonomously manages a varied and complex caseload of adult dietetic referrals with nutritional support needs working as part of the community team and wider multi-professional team to provide specialist dietetic advice in relation to nutritional management and to produce a coordinated care plan of care. Leads on embedding and strengthening nutritional screening at the 'front door' and as part of rapid response pathways and models of care including providing training and education, including accredited training to therapy assistant practitioners and others working as part of the front door model to de-escalate risk

Supports the wider community nutritional support team by providing operational leadership alongside other community leads, ensuring effective prioritisation of referrals and that dietetic pathways and processes are fully embedded.

Delivers dietetic care in a variety of settings to meet the needs of the patient including undertaking home visits, attending care home/residential settings, community hospitals and providing group education and working across virtual platforms.

Person Specification

Qualifications and knowledge

Essential

  • Recognised Degree in Dietetics
  • HCPC Registration
  • Relevant post graduate or Masters level qualification
  • Demonstrates consolidated and broad clinical dietetic knowledge and skills in adult dietetics
  • Critical appraisal skills: knowledge of current and emerging evidence base, national and professional policy, guidance and best practice in the delivery and development of broad spectrum of adult dietetics
  • Well established clinical problem solving skills
  • Demonstrates effective organisational skills including prioritisation of workload, effective caseload management and delegation of tasks
  • Well established teaching and presentation skills
  • Evidence of attendance on accredited specialist courses and /or advanced training in relation to adult dietetics
  • Demonstrates good knowledge of, and ability to apply, evidence base in relation to dietetic practice and development of practice


Desirable

  • Clinical supervisory skills training
  • Completed BDA specialist accredited Dietetic group training /modules
  • ECDL
  • Other professional technical or academic qualification which relate to area of clinical dietetic practice


Experience

Essential

  • Demonstrable and considerable experience in adult clinical dietetics
  • Experience of working in acute and community settings to deliver dietetic care
  • Evidence of working as an integral part of of multidisciplinary teams
  • Experience of teaching other health care staff
  • Evidence of involvement in development work in dietetic services e.g audit, implementing a change in practice, delivering a new dietetic intervention
  • Experience of clinical audit

  • Experience of research

  • Experience of delegating work to and supporting / supervising D.A. / students / junior dietetic staff
