The post holder will be an active member of the multidisciplinary team within Children's Services by using professional judgement and assuming responsibility and accountability for assessing, implementing and evaluating clinical care for patients to meet their needs in a safe caring environment that promotes dignity, privacy and respect.
The post holder will be expected to develop knowledge and skills within the speciality and will assist in the managment and organisation of nursing work in the Team within the NMC Code of Professional Conduct.
Main duties of the job
The community staff nurse will be attached to a caseload of patients . This role will provide the opportunity to work closely with GP practices and Trust Consultants in supporting patients with highly complex and often high acuity needs to prevent unnecessary hospital admission. This role requires an clinician with excellent, team working and communication skills, to provide patient centred careand delivering effective evidence based care to a defined demographic population with diverse health care and social needs.
The successful candidate need to be prepared to adapt to the changing needs of the Trust and the community models of care as they arise.
