Are you a qualified Learning Disability or Adult Nurse (or other nursing qualification with experience of working with people with Learning Disabilities)? Due to an increase in demand we are looking for clinicians to join our team!

An opportunity has arisen for a Band 5 Learning Disability or Adult Nurse within Portsmouth's Integrated Learning Disability Service (ILDS) hosted by Portsmouth City Council where our service has been rated as 'Outstanding' in the last 2 CQC inspections.

As a Nurse in our Complex Health Team, you will provide person centred assessments and interventions for adults with Learning Disabilities who have associated complex health needs in partnership with other agencies and carers which form part of our Multi-disciplinary Team. We need you to be a strong communicator, able to adapt to our patients unique needs as well as external agencies you may be linking with. You will be able to work autonomously so time management and organisation skills are essential. Our patients have slightly higher and more complex healthcare needs and they may be funded by CHC.

We are looking for candidates who hold a professional registration with the NMC and who hold a full UK driving licence and have access to the use of a car.

Main duties of the job

*To provide specialist services to adults with a learning disability. This is achieved through a process of Person-Centred Care, which is comprehensive and responsive to the needs of the individual and their family/carers. Identified needs will be assessed, treatments implemented and evaluated in partnership with Service Users as well as family/carers.

*To support the Integrated Learning Disability Service in meeting the healthcare needs of people with learning disabilities in Portsmouth City via: -

*Improving access to mainstream healthcare services

*Supporting the development of enhanced, or adapted, mainstream healthcare services

*The provision of appropriate information to Service Users and their carers.

*Facilitating the increased involvement of Service Users in healthcare services

*The provision of some specialist services

*Due to the inter-connected nature of the Integrated Learning Disability Service, there is an expectation that all staff will be prepared to support any elements of its service as needs dictate and in line with their professional competencies.

Person Specification

Qualifications

Essential

  • RNLD or RGN (with appropriate experience and knowledge) or equivalent essential -- (other parts of the Nursing Register may be considered dependant upon relevant experience)
  • ENB 998 or equivalent in teaching & assessing
  • Full clean driving licence


Desirable

  • Degree in Nursing
  • Other relevant post graduation or specialist training


Experience

Essential

  • Experience of working with People with a learning disability, within a variety of settings.
  • Experienced in developing care plans
  • Experienced in developing and delivering care and clinical interventions to patients
  • Experience of MDT working
  • Experience of teaching, training and / or supervision
  • Evidence of recent CPD
  • Evidence of developing policy, guidelines contributing to management of resources
