An opportunity has arisen for enthusiastic staff nurses to join Tim Dyffryn Clwyd, a Community Mental Health Team, providing support for people with a serious mental illness in Denbighshire.
This is a fixed for 9 months to cover maternity leave.
The ability to speak Welsh is desirable for this post Welsh and/or English speakers are equally welcome to apply.
Main duties of the job
The team provide support and interventions for Adults from aged 18 to 70.
The post holder is required to work within an integrated, multi-disciplinary team focusing on adults with severe and enduring mental health problems. The post holder will undertake the responsibility for the care co-ordination and case management of a defined caseload of clients with complex needs, as defined by the Mental Health (Wales) Measure 2010
