We are inviting applications for motivated & dynamic Community Practitioners (nursing and AHP) to support/complement our committed team of registered professionals within North Tees & Hartlepool Foundation Trust's Care Group 1. Working autonomously as a Practitioner under the 2 hour community crisis response, the post holder will have responsibility for reactively assessing, prescribing, monitoring & managing patients, recognising the early symptoms of disease exacerbation, acute illness & injuries based on the understanding of long-term conditions, disease processes, current evidence and practice standards. The service is delivered to a population of patients within the community setting residing in their own or residential care homes across both the Hartlepool and Stockton localities. The key aim of the urgent two hour community crisis response is prevention of inappropriate hospital admission and providing timely support to patients. In achieving these outcomes there will be a reliance on comprehensive physical assessment and treatment-planning including prescribing along-side the wider community teams.

Main duties of the job

Working autonomously as a highly skilled Practitioner, the post holder will have responsibility for reactively monitoring/managing patients, recognising early symptoms of disease exacerbation, acute illness & injuries based on the understanding of long-term conditions, disease processes, current evidence & practice standards. . The key aim of the service is prevention of inappropriate hospital admission/readmission.

Post involves liaising with health/social care providers & other agencies to ensure provision of high quality crisis support to this group of patients/carers. Post-holders will also forge strong links with the clinical triage and ISPA team, community hub, CIAT and Rapid Response Teams as well as PCN's within the locality.

The Community Practitioner service operates over 7 days a week, covering the hours of 08.00-20.00hrs across the Hartlepool and Stockton localities.

The Community Practitioner service is a team of highly skilled advanced nurse/AHP practitioners working within Care Group 1 of the Trust (Healthy Lives) . The practitioners are hold the Non Medical Prescribing qualification.

The practitioner deliver reactive & proactive interventions to people residing in care homes across the Hartlepool & Stockton CCG area.

The key focus of this team is to ensure that people are cared for in their own home environments & that hospital admission is prevented where possible.
