We are looking for a motivated, qualified and registered professional who has significant experience within adult mental health services and/or experience of working with children and young people, preferably within a community CAMHS service.

The role is based within the Kingswood Civic Centre with the South Gloucestershire Recovery (South) Team. The role covers the whole of South Gloucestershire. The caseload is small to reflect the complexity of the work involved.

Main duties of the job

To establish and support a transitions pathway between CAMHS and adult services

To identify young people within CAMHS experiencing emotional dysregulation and work alongside them to support transition into adult services

To provide specialist advice to young people, family and professionals

To support planning of specialist programmes of care

To review transition process after 9 months and identify notable practice or areas for improvement

Working with young people experiencing emotional dysregulation in South Gloucestershire

Ensuring a cohesive, smooth transition from CAMHS to adult services (AMHS) for people experiencing emotional dysregulation is evidenced as being significant in terms of their longer term recovery and is the driver for this role. Also, NICE guidelines detail good practice in respect to the transition process and these will inform practice.

Work will be undertaken in both CAMHS & AMHS, alongside interface with psychological therapies in adult services.

Job description

Job responsibilities

1.To work as a member of the South Gloucestershire adult recovery service but also specifically link into, and share a base with, the Child and Adolescent Mental Health Service.

2.To provide comprehensive and specialist advice around the development of a care pathway for a young person experiencing emotional dysregulation who is transitioning into AMHS

3. To work with professionals and carers for the young person to ensure transition is seamless

4.To plan, prioritise, and work alongside the young person to ensure the transition process is clearly understood and managed effectively

5.To ensure that a care coordinator is identified within AMHS and that joint working is undertaken during the period of transition

6. To ensure that any other multi-agency input is in place to meet the young person's needs

7. To work with and on behalf of young people in a way that promotes and develops their efforts, resources and quality of life. To deliver services in a manner that is appropriate, relevant and respectful to all community members, recognising diversity of ethnicity, culture, belief, privilege and capacity.

8. To liaise and work in partnership with staff from other statutory services, voluntary agencies and community groups that work with adolescents,

9. To facilitate and improve pathways from third sector/statutory services to relevant adult and

10. To provide advice and to assist other professionals working with young people with interventions which draw on own specialist professional knowledge.

11. To provide advice and support to carers/ relatives of young people,

12. To participate in clinical meetings g. CPA and client reviews as appropriate.

13. To support the full implementation of the Care Programme Approach.

14. To ensure that legal and Trust requirements affecting service user and staff are adhered to, and ensure a safe working environment for service users and staff. To cooperate with statutory agencies with the responsibility to protect children and ensure that issues of risk and safety of young people are assessed and any necessary action is implemented.

15. To maintain a high degree of professionalism at all times when working with highly distressed people

16. In line with Trust policy, to receive regular clinical supervision. To also participate in caseload supervision arrangements provided by the Wiltshire Early Interventions for Psychosis Service.

17. To maintain accurate records relating to clinical and professional activity and to monitor clinical workload using agreed systems.

18. To communicate complex and sensitive information to patients, carers and professionals, including information about diagnosis and prognosis, to find ways of enabling young people and their families to take this information on board, and to provide empathy and reassurance.
