If so Mental Health Services for Older People (MHSOP) are looking for someone who possess confidence, enthusiasm, initiative and a positive approach to teamwork but most importantly have a genuine passion for patient care.

MHSOP have exciting plans to modernise our workforce to meet current and future challenges and provide a new career pathway for non-medical practitioners through the introduction of Advanced Clinical Practitioners into our community services.

This role will be based within the City Community Mental Health Service, based at Highbury Hospital.

The service incorporates both Community mental Heath team and the Intensive Home Treatment Team, providing both urgent care and ongoing routine provision to older adults with both Serious Mental Illness and Dementia.

It is anticipated that this role will develop over time in response to changes in service delivery and therefore we need clinicians who are energetic, eager to develop and are forward thinking with the potential to adapt to change and meet new challenges. We will ensure we support you at every level to enable you to develop the role within MHSOP services.

Main duties of the job

The post holder is an experienced Nurse/Allied Health Professional/Paramedic, qualified to ACP Masters Level who will act as a Clinical Leader. They will:
  • Demonstrate safe , senior clinical decision-making by autonomously assessing patients presenting to the inpatient service including initial history taking, clinical assessment, diagnosis, treatment and evaluation of care
  • Demonstrate critical thinking in the clinical decision making process and work collaboratively with the MDT to meet the needs of patients.
  • Be instrumental in guiding junior medical staff and nursing staff in relation to relevant protocols, procedures and clinical pathways.
  • Be fundamental in the development of this new role.
  • Initiate, manage and drive innovative change in practice for the benefit of patient care in line with Trust and local procedures.

The role will be an integral part of the multidisciplinary team. The Advanced Clinical Practitioner will work within a defined scope of practice and to agreed clinical standards applied within the service.

The role will be multi factorial including clinical assessment, formulation, non-medical prescribing (where appropriate) and developing holistic non-pharmacological treatments.

The ACP may provide mentorship to trainee ACP's within the directorate.

1st on call duties on a 3 month rota basis, will form part of this role.

Job responsibilities

  • Complete a full system and physical or psychological/psychiatric exam
  • Take a systemic and/or psychosocial patient history
  • Make diagnostic decisions based on consultation, history, presentation, pathology and imaging results if required.
  • Initiate, prescribe and monitor the impact and outcome of treatments and/or medication including efficacy, side effects, contraindications and risk
  • Devise an individual clinical management plan that includes all aspects of multi-disciplinary care.
  • Screen patients for early signs of disease and risk factors and relapse indicators
  • Carry out, if appropriate to role, invasive and non-invasive diagnostic and therapeutic procedures with due regard to mental capacity and advance decisions
  • Refer to other health professionals as appropriate


Person Specification

Qualifications

Essential

  • Current Registration with NMC or HCPC
  • Clinical Based Degree
  • MSC Advanced Practice


Desirable

  • Management/Leadership Experience
  • Recognised Teaching or Mentorship Qualification
  • Non-Medical Prescriber
  • Experience of working in Mental health and/or Dementia


Experience

Essential

  • Experience of working with older patients who have complex physical and mental health needs.
  • Demonstrate ability to work across all 4 pillars of 'Advanced Clinical Practice'
  • Experience of working with Older People with Mental Health issues & Complex Physical Comorbidities.
  • Evidence of extensive expert professional/clinical knowledge managing unwell patients in the community, primary or acute setting.
  • Specialist clinical training and CPD in relation to patient history taking, clinical examination and developing management plans for patients. Knowledge of national and local NHS healthcare agenda, standards, research and its implications.
  • Participation in research, audit and evaluation
  • Knowledge and experience of chronic disease management and long-term conditions.


Desirable

  • Experience of working as a senior clinician
  • Experience of involvement in policy and practice development
  • Computer literacy
