Are you a Registered Nurse with an interest in Long Term Conditions seeking an exciting opportunity to work within the North East Derbyshire Enhanced Community Practitioner Team?
If so, this opportunity is for you!
The Enhanced Community Practitioner (ECP) Service in North East Derbyshire is made up of a team of trainee and qualified ECP'S and Band 5 Long Term Conditions Nurses.
The post is to support the patients within the Clay cross and North Wingfield area.
The service works very closely with the DCHS Integrated Community Services (Nurses, Physiotherapists and Occupational Therapists), the GP Practice Staff, Specialist Nurses and Social Services to ensure delivery of the NHS Long Term Plan, supporting the Ageing Well Programme.
The primary aim of the service is to support people with Long Term Conditions to Live Well for as long as possible in their own homes (or place they call home) and avoid unnecessary hospital admissions.
Main duties of the job
The Service is to provide holistic assessment and care planning for people with Long Term Conditions, supporting them to pro-actively manage their conditions, putting their wishes and expectations central to their care.
Patients are seen in their own homes and Residential Care Home settings, to prevent hospital admissions or facilitate early discharge.
The service uses a blended approach of face to face and virtual appointments to optimise effective provision of care.
The Band 5 Long term Condition Nurses work alongside the Enhanced Community Practitioners to educate and support patients and their family/carers to optimally manage their conditions, minimise avoidable deterioration, as well as supporting those patients who are approaching the end stages of their condition.
About us
There has never been a more exciting time to join DCHS, one of the largest and constantly expanding specialist community NHS providers in the country - one that is also among the very best NHS employers, as voted by its own staff and rated as outstanding by CQC.
Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.
We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people's own homes and via virtual consultations.
During the pandemic response we have led the implementation of public vaccination centres in collaboration with partners across health and social care.
