Preston Park Community PCN Care Home & Frailty Service (in partnership with Sussex Community NHS Foundation Trust SCFT) are pleased to offer an exciting opportunity for a band 6 Occupational Therapist to join the new and innovative Frailty Service. We are a forward thinking, evolving patient-centric and busy Care Home & Frailty Service, covering Preston Park Community PCN, based in the city of Brighton & Hove. We are actively improving access to healthcare for those most at need either living in care homes and those housebound.

As a band 6 occupational therapist, working in Primary Care, you will be a key member of Preston Park Community Frailty multi-professional team, providing intervention to persons considered frail or pre-frail within both the community and care home settings. You will be key in improving the functional outcomes for this client group and using your specialist skills to provide holistic assessments and intervention. You will install personalisation, improve participation and purposeful occupation.

As this is a new post, you would be actively involved in the exciting development of this service by sharing your ideas and providing professional expertise to develop and enhance the care offered as part of the multi-disciplinary team.

Full time or part-time as a job share would be considered.

The post is a 2 year fixed term contract.

Main duties of the job

  • You will provide occupational therapy intervention to a defined frailty caseload, working collaboratively with team members and in partnership with wider support services.
  • Work proactively with patients in the self-management of their conditions, through promotion of healthy lifestyle choices and facilitating engagement in meaningful occupations.
  • Complete a holistic assessment including physical and mental health approaches and development of an intervention plan, including joint goal setting with the patient that facilitates personalisation and occupational engagement.
  • Completion of onward referrals and signposting to appropriate health, social care and voluntary sector services, including integrated approach with social prescribers
  • Support the Advanced Care Planning processes for the frail population
  • Support carers, families, and communities through identification of carer's needs, shared decision making and signposting to enable early support provision.
  • Promote the value of occupational therapy within Primary Care and partnership organisations.

Job Summary
  • Working as part of Preston Park Community PCN Care Home & Frailty Service, as a key member of the multi-professional team, providing intervention to persons considered frail or pre-frail within both the community and care home settings.
  • Provision of occupational therapy intervention in line with agreed professional and organisational standards and policies to improve the functional outcomes and occupational participation for this client group.
  • To devise support plans, implement and evaluate occupational therapy treatment plans utilising specialist and enhanced knowledge and skills focusing on the allocated care home population and the PCN community population.
  • To work proactively with community patients in the self-management of their conditions, through promotion of healthy lifestyle choices and facilitating engagement in meaningful occupations.
  • To complete a holistic personalised assessment incorporating physical and mental health approaches to develop an intervention plan, including joint goal setting with the patient that facilitates occupational engagement.
  • To use motivational interviewing techniques, negotiating skills and patient centred goal planning.
  • Work with partner organisations across the network to achieve the best outcomes for patients.

  • Communication and Working Relationships
  • To build professional relationships with care home staff and provide specialist advice to support the care of their residents
  • To build professional rapport with patients and their families, communicate with them in a way that respects their views, autonomy and culture, ensuring that they are involved in and informed about their care packages
  • To develop key partnerships, working to ensure effective whole system working, through effective communication and networking with other disciplines and by working across professional boundaries
  • To communicate effectively with all professionals concerned with the patients care

  • To report effectively to the practice and multi-disciplinary team on patients progress and performance in relation to the patient goals

  • Key Responsibilities
  • To provide and develop information for others about occupational therapy, particularly in own specialist are
  • To be aware of and adhere to requirements of risk management policies, quality standards, clinical audit and involvement in the provision of outcome measures in line with clinical governance requirements
  • Supervision and Appraisal - Review and reflect on own practice through regular participation in clinical supervision and annual appraisal with the Occupational Therapy Professional Lead/Operational Manager
  • Training staff and students -To take a lead on planning and delivering training on own specialist area, within primary care network and evaluate the effectiveness of the training provided
  • Professional development - To maintain professional registration as an occupational therapist, with the HCPC, and show proof of registration as requested
  • Continuing Professional Development - To undertake professional training that may be required by the Trust and to undertake relevant activities to meet the objectives identified with your clinical and line managers
  • Personal management - To exercise good personal time management, punctuality and consistent reliable attendance
  • Quality Improvement - To take part in service evaluation service audit and outcome measurement to ensure that quality is maintained
  • Best Practice To keep up to date with current guidelines relating to speciality to insure best practice is maintained
  • To role model occupational therapy as a profession, communicating and showcasing its value and contribution to primary care
  • Undertake clerical duties as required including inputting of information on to IT databases.

  • Main Tasks
  • To plan therapy, social and rehabilitative pathways taking account of the fluctuating needs of the patient and assist in evaluating the outcomes, focusing on the pre-frail population
  • To work effectively as part of the primary care network in providing and facilitating a high standard of care, social and rehabilitative support in the community, particularly the pre-frail population
  • To provide specialist occupational therapy interventions and holistic care for the referred patient group
  • To report and document variances in a patients condition or planned pathway of care, as part of a multi-disciplinary approach
  • To offer advice and support and education on healthcare to both patients and carers
  • To undertake a holistic assessment of all the patients needs and understand the routes for escalating issues that are not directly resolved by own professional practice
  • Formulate and deliver an individual treatment programme based on a sound knowledge of evidence-based practice and treatment options using specific clinical assessment, reasoning skills and knowledge of treatment skills
  • To evaluate patient progress, reassess and alter treatment programmes as required
  • Assess patient understanding of treatment proposals, gain valid informed consent and have the capacity to work within a legal framework with patients who lack capacity to consent to treatment
  • To undertake advanced care planning and work towards achieving the best outcomes for patients and their carers
  • To manage clinical risk within own patient case load and recognise potential risks to others, complying at all times with the requirements of health and safety regulations.

    Qualifications

    Essential

    • oProfessional qualification such as a degree or equivalent in Occupational Therapy. oCurrent registration with the Health and Care Professions Council as an Occupational Therapist. oEvidence of relevant postgraduate courses / CPD and reflective practice relevant to clinical speciality oEvidence of a range of clinical competencies appropriate for the role

    • oSubstantial relevant post-qualification experience including working with frail and pre-frail client groups oExperience of interdisciplinary working oExperience of supervising junior staff, assistants and students oExperience of independently managing a clinical caseload oContributions to audit and evaluation within a clinical governance framework oExperience of a range of caseload management and prioritisation systems
