Norfolk Community Health and Care NHS Trust are looking for 2 Registered Nurses to join the Community Access Team (based at the Norfolk and Norwich University Hospital). In November 2020 this team was formed following the merger of the Early Intervention Team and Community Liaison Team. The team support the admission avoidance pathways in A& E and the Older Persons Emergency Department, as well as discharge pathways from the NNUH via D2A1 and D2A2 pathways, assessing patient's suitability for community hospital beds for either rehabilitation, Reablement, a health need or End of Life care
Main duties of the job
The service runs seven days a week, shifts 8am to 8pm (Admission Avoidance), & (Bed based transfers from the acute) Mon - Fri 8am to 6pm Sat/Sun 8am to 4pm. The team consists of Nurses, Occupational Therapists, Physiotherapists, and Therapy Assistants. At the front door (Admission Avoidance aspect of the service) the team operates against strict A+E waiting targets and the successful candidate must have excellent written and verbal communication, as well as strong clinical reasoning skills. The ability to complete quick and decisive assessments is essential.
The 'back door' - bed based aspect of the service, delivers safe and timely transfers of patients from acute Hospitals or Admission Avoidance from home into the NCHC Community inpatient units across Norfolk. Working alongside Community Inpatient Unit Ward Managers.
We are a small team, who are very supportive of each another and there are opportunities for Continuing Professional Development, in the form of study days. We see and assess patients with a wide variety of conditions, including long term disabilities and neurological conditions.

You will be providing a patient focused service ensuring smooth planning, coordination and communication between services in order to facilitate seamless care and identification of gaps in the present systems.
You will link directly with wards, acting as a resource to the multi-disciplinary team, patients, and their carers, in order to support safe and timely discharge.
You will be working across organisational boundaries, ensuring open, honest and effective communications are maintained in order to facilitate joint working practices around integrated patient pathways for transfer of care
You will work proactively and positively with colleagues within the Integrated Discharge Team to eradicate delays in transfers from the Acute sector.
You will be working with Emergency areas and Intermediate Care to provide holistic patient assessment in order to avoid admission for patients who do not require acute intervention.
